To ensure a smooth and efficient experience at JBDAPH, patient scheduled for Imaging (CT Scan/Ultrasound) or Functional Tests (PFT/Stress Test) are required to present the following:

𝟭. 𝗗𝗢𝗖𝗧𝗢𝗥’𝗦 𝗥𝗘𝗤𝗨𝗘𝗦𝗧
Must clearly specify the needed exam:
• Imaging: CT Scan, General Ultrasound, or OB Ultrasound.
• Pulmonary/Cardiac: Spirometry (PFT) or Stress Test.

𝟮. 𝗖𝗛𝗔𝗥𝗚𝗘 𝗦𝗟𝗜𝗣 & 𝗗𝗜𝗔𝗚𝗡𝗢𝗦𝗜𝗦
• Bring the validated charge slip for your specific test.
• Ensure your initial diagnosis is written down (you may ask a Nurse or Nursing Attendant for assistance with this).

𝟯. 𝗩𝗔𝗟𝗜𝗗 𝗜.𝗗.
Present at least one: National ID, Postal ID, Barangay Certificate, MSS ID, or any government-issued ID.

Ito ay upang masiguro ang kaligtasan at mapanatili ang kalusugan.
