💠 4 Copies Brgy. Indigency sa Persons With Disability (kung Guardian or Parent ang mu process ijang name sa Indigency),
💠 4 Photocopies Persons With Disability ID Card &
💠 4 Photocopies Valid ID sa Guardian or Parent if silay mu process
