Philippine Health Insurance Corporation (PhilHealth) reminds all members who wish to transfer their YAKAP Clinic to visit the nearest PhilHealth Local Health Insurance Office (LHIO) until Friday, December 26, 2025. This is to ensure continuous access to benefits under the primary preventive care benefit.
To transfer to another YAKAP Clinic, whether or not a First Patient Encounter (FPE) has occurred, the member or their dependent must go to the nearest LHIO and submit a completed Primary Care Selection Form (PCSF). Members will also undergo verification using the PhilHealth Check Utility (PCU). PhilHealth will notify members via email about the transfer details, which will take effect upon approval starting January 1, 2026.
PhilHealth allows transfers through an authorized representative, provided they bring an authorization letter, a valid government-issued ID (or equivalent document), and a completed PhilHealth Member Registration Form (PMRF) or PhilHealth Claim Form (PCF), if required.
The state health insurer also reminds members that those who have not yet had an FPE may request to transfer providers at any time. Once approved, these transfers take effect immediately. For those who have already undergone an FPE, transfer requests can only be made during the fourth quarter of the year (October to December).
Members can view the list of YAKAP Clinics at www.philhealth.gov.ph to choose a facility near their location. The list is regularly updated with newly accredited YAKAP Clinics and contact details.

Through PhilHealth YAKAP, members and their dependents can avail of primary care health services, such as regular check-ups, basic laboratory or diagnostic tests, cancer screening, and medicines, which will be added with 54 more under PhilHealth Guaranteed Accessible Medications for Outpatient Treatment (GAMOT). # # #
