Shoulder balance is a crucial aspect of scoliosis surgery, influencing patient appearance and satisfaction after corrective procedures. While advancements in spinal instrumentation have improved curve correction, achieving and maintaining shoulder balance remains a key goal. Postoperative shoulder imbalance can lead to dissatisfaction and, in some cases, require further intervention. Here's a case of syndromic scoliosis (Marfan's syndrome) that we managed with correction of shoulder balance and posterior spinal fusion from T4-L3. Achieving and maintaining shoulder balance is a complex process that requires careful consideration of various factors during scoliosis surgery. Surgeons need to balance curve correction with the need to maintain or restore shoulder balance to ensure optimal outcomes and patient satisfaction🙏🏻

Cervical spinal manipulation therapy (CSMT) by chiropractors has been temporally associated with VAD (vertebral artery dissection). While causality is debated, multiple case reports and retrospective studies suggest an association, especially in young patients presenting with stroke-like symptoms post-manipulation. Neck rotation and extension during manipulation can stretch the vertebral artery, especially at the C1–C2 level. Chiropractors and physicians should screen for red flags (sudden neck pain, headache, dizziness). Avoid high-velocity neck manipulation in patients with suspected or unexplained neck/head pain. Early referral for imaging is warranted if symptoms suggest possible VAD.

June is recognized as Scoliosis Awareness Month, and is dedicated to raising awareness about scoliosis, to promote early detection and treatment. Sharing a case we did of AIS (adolescent idiopathic scoliosis). We performed corrective surgery with posterior intrumented fusion from T3-T12 (selective thoracic fusion) Its important to remember that the goal of any scoliosis surgery should be to prevent progression of the curve, achieve a well balanced spine, and perform a stable fusion involving the least number of segments possible🙏🏻
