👁️ Looks similar, but not the same! Which is which?
Pterygium vs Pseudopterygium: Orientation & Attachment Matter!

🔸 Pterygium
➡️ Grows horizontally, most often nasally
➡️ Invades Bowman’s layer, firmly adherent across the limbus
➡️ Often UV-related and progressive

🔹 Pseudopterygium
➡️ Can appear anywhere, often oblique
➡️ Not truly adherent to the limbus — only at the apex
➡️ Caused by trauma, surgery, or inflammation

Bowman’s Probe Test:
If you can pass a probe under the apex → it’s a pseudopterygium (positive test)
If it’s firmly attached → likely a pterygium

👁️✨ Limbal Dermoid Removal for Better Vision!
A limbal dermoid is a benign choristoma—a non-cancerous growth made up of normal tissue in an abnormal location—typically found at the junction of the cornea and sclera. Though harmless, it can distort the shape of the eye, leading to hyperopia and irregular astigmatism.

In this case, the patient couldn’t tolerate glasses due to the corneal distortion. 👓❌ I performed surgical excision, and after full healing, the patient will be able to wear contact lenses to correct the irregular astigmatism and improve vision. 👁️💫

This is the double ring sign after a complete air fill in DSAEK. Just in case you didn’t mark and you want to confirm if your graft is properly oriented, the inner ring should be brighter/closer to you compared with the outer ring. Because if you punch the graft during preparation, the endothelial portion (outer ring is larger in size), thus farther/deeper than the stromal side of the graft.

Fluorescein highlights the ulcer, rose bengal reveals the enemy’s edge – the devitalized cells and terminal bulbs. Fluorescein staining highlights areas of epithelial loss, revealing the central dendritic ulcer characteristic of HSV. Rose bengal, on the other hand, stains devitalized and infected epithelial cells—particularly at the terminal bulbs—providing critical information on lesion activity and margins.
When HSV is suspected, fluorescein is used first to see the classic dendritic pattern, then rose bengal can be applied to better delineate infected margins. 📸 R1MC residents

Staph Blepharokeratoconjunctivitis with Ruptured Descemetocele
A high-risk case managed with cyanoacrylate glue application and bandage contact lens. Timely intervention preserved the globe integrity and stabilized the cornea, with UCVA 20/20. It is very important to treat the root cause which is blepharitis, patient is on long term blepharitis treatment with warm compress, lid scrubs, and oral Doxycycline for flare ups.
If you have recurrent stye, lid redness, itchiness, consult your Ophthalmologist to treat the Blepharitis before it’s too late.
💉 Dx: Staph BKC with corneal melt
🩹 Tx: Cyanoacrylate glue + BCL

This patient was referred to me for scleral melt after bare sclera pterygium surgery.
I performed a scleral patch graft to cover the area of scleral thinning, followed by a conjunctival autograft to cover the previously exposed scleral surface
👀👁️Always remember: It is important to emphasize that in pterygium surgery, the use of a conjunctival autograft is critical in minimizing postoperative complications such as scleral melt, recurrence, and delayed healing. The bare sclera technique, though simpler, is associated with higher risks and is generally discouraged in current surgical practice.

✨ Vision Restored Without Surgery!
A patient came to me with blurred vision (20/150) due to a corneal scar from a past infection. Instead of going straight to a corneal transplant, we tried a rigid gas permeable (RGP) contact lens—and it worked wonders! 👁️💫

The vision improved to 20/30, and surgery is no longer needed.
Sometimes the simplest solution makes the biggest impact! 🙌
