Conjunctival autograft is the gold standard to keep it from coming back, and with fibrin glue (sutureless), it’s quicker, smoother, and pretty much painless.

A young female patient came in with redness in the left eye — but on exam, conjunctival papillomas were found in both eyes. Conjunctival papillomas are benign, often HPV-related growths on the conjunctiva. They appear fleshy, vascular, and lobulated, and may be pedunculated or sessile. Though typically non-cancerous, they require excision and biopsy to rule out dysplasia.

👩‍⚕️ The patient opted to remove the more symptomatic left eye lesion first, with the right eye scheduled for surgery soon.

This patient had a primary pterygium excision without graft at another institution and developed a conjunctival pyogenic granuloma (or lobular capillary hemangioma) with corneal dellen. I excised the granuloma. Granuloma is an uncommon but known complication—more likely when no graft is done due to delayed healing over area of bare sclera, exposed residual tenon’s capsule, or suture reaction. Studies have shown that techniques with grafts significantly reduce both recurrence and inflammatory complications like granulomas.

Pterygium vs Pseudopterygium: Orientation & Attachment Matter!

🔸 Pterygium
➡️ Grows horizontally, most often nasally
➡️ Invades Bowman’s layer, firmly adherent across the limbus
➡️ Often UV-related and progressive

🔹 Pseudopterygium
➡️ Can appear anywhere, often oblique
➡️ Not truly adherent to the limbus — only at the apex
➡️ Caused by trauma, surgery, or inflammation

Bowman’s Probe Test:
If you can pass a probe under the apex → it’s a pseudopterygium (positive test)
If it’s firmly attached → likely a pterygium
