✅1ST DOSE: MODERNA/ JANSSEN/ SINOVAC
(Walk-in: open to all kahit taga ibang lugar)

✅12-17 yrs old: 1st & 2nd dose (walk-in kahit taga ibang lugar)
Vaccine: PFIZER

TIME: 8AM-2PM
VENUE: MALOLOS SPORTS AND CONVENTION CENTER beside NEW MALOLOS CITY HALL

PLEASE BRING THE REQUIREMENTS:
ADULT: 1st dose VALID I.D

ADULT BOOSTER DOSE: VALID ID, VACCINATION CARD

🖤BIRTH CERTIFICATE/ BAPTISMAL/ FORM 137
🖤VALID ID, SCHOOL ID
🖤 1 VALID ID OF PARENTS/ GUARDIAN
🖤 Authorization letter & Certficate of Barangay for legal guardian ( no parent/s available)

For those with comorbidity (e.g. asthma, g6pd, heart prob etc.) We also require medical certificate from their doctor.

Valid ID (parent)

Valid ID, parent authorization, Barangay certificate of guardianship (guardian)

*Guardian ay dapat 21 y/o and above
*2nd dose-vaccination card

Kumpletuhin ang ikalawang dose ng COVID-19 vaccine para sa pinalakas na proteksyon!

Hindi kumpleto ang kalahati!

Ang mga bakunang Sinovac CoronaVac at Oxford-AstraZeneca ay ibinibigay ng dalawang (2) doses. Kailangan mong makuha ang parehong dose para makuha ang pinakamataas na lebel ng proteksyon mula sa COVID-19!

Bilang ng dose na kailangan at pagitan:
✔️ Oxford AstraZeneca: 2 doses, 4-12 na linggo ang pagitan
✔️ Sinovac CoronaVac: 2 doses, 28 days (4 na linggo) ang pagitan
