This announcement is for ALL INCOMING FIRST YEAR AND TRANSFER STUDENTS of Capiz State University – Mambusao Satellite College.

Before proceeding with your enrollment, please make sure you have completed and submitted all required medical documents.*🩺📝

🔍 Follow the instructions carefully to avoid any delays!

📌 You need to prepare and submit:
✅ Your laboratory test results
✅ A medical certificate from your preferred physician
✅ One (1) 2x2 ID photo

📁 Place all documents in a long brown envelope and submit them to the campus clinic nurse.
