Step 1.
Call or Leave us a message on our page, Email, and Mobile Health Hotlines through the following numbers and one of our Mobile Health staff will be ready to assist you:

Step 2.
Provide the name of your preferred doctor / specialist so we can check their availability.

Step 3.
If your doctor is available for Telemedicine, our staff will require you to provide the following personal information relevant for Teleconsult:

✔️Complete name of the Patient
✔️Date of Birth and Gender
✔️Contact Number
✔️Email address
✔️Preferred Doctor
✔️Preferred Date and Time of Appointment
✔️Chief Complaint/ Reason for Consultation
✔️Mode of Payment (Bank Transfer / GCash)

**Note that your information will be saved for your Electronic Medical Record and be kept with utmost confidentiality**

Step 4.
Our Mobile Health staff will inform you if your doctor agrees or is available on your preferred Date and Time. Otherwise, another Doctor on Deck will be suggested as an option.

Step 5.
Once your appointment has been confirmed, an Invitation and Link for Teleconsultation will be sent to the email you provided.

Step 6.
Payment options will be provided.

Prepare for your Teleconsult.
