GE TMX common B4 charger IGBT failures. The IGBT collector PCB trace is very close above the gate - emitter resistor thus when presence of dirt - moisture that leads to small arcing.
Best is to put mylar insulation as shown below

Initial report that there was an accident that caused damages to both the RCB and Main Contactor and was replaced by the previous service provider.
It was said that the original problem was a Communication Problem between console and generator thus 2 MCB were orderd to resolve problem this.
During our trouble shooting we found that the +24VAA got shorted by the Enode Board and that caused damage to the MCB copper trace that cut connection to the +5V ISO regulator that inhibits ON/OFF function.
We found a shorted back diode in the Enode Board
Both the MCB damaged copper trace was fix as well as the Enode shorted diode was remove. A missing back diode inthe Main Contactor 24V DC coil added. System was down for a year.
