💧 Hydration is Key

* Drink water regularly—even if you’re not thirsty
* Aim for 2–3 liters/day (or as advised by your doctor)
* Avoid sugary drinks and excessive caffeine



🌡️ Avoid Heat Stress

* Stay indoors during peak heat (10 AM – 4 PM)
* Use fans or air conditioning
* Wear light, breathable clothing



🧴 Protect Your Skin

* Use SPF 30+ sunscreen (reapply every 2 hours)
* Wear hats, sunglasses, and long sleeves when outdoors
* Transplant medications increase sun sensitivity ⚠️



💊 Take Medications Properly

* Never skip or delay your immunosuppressants
* Store medicines away from heat and direct sunlight
* Keep a consistent medication schedule



⚠️ Watch for Warning Signs

Seek medical help if you experience:

* Dizziness or fainting
* Decreased urine output
* Rapid heartbeat
* Excessive sweating or confusion

Dialysis Access is Your Lifeline ❤️

If you are on hemodialysis, your dialysis access is the place where blood leaves your body, is cleaned by the dialysis machine, and returns to you.

Without a working access, dialysis cannot be done. That is why we call it your lifeline.



Types of Dialysis Access

1. Arteriovenous Fistula (AV Fistula)
• A connection between an artery and vein created by surgery
• Usually placed in the arm
• Best and safest long-term access

2. Arteriovenous Graft (AV Graft)
• Uses a synthetic tube to connect artery and vein
• Used when veins are not suitable for fistula

3. Central Venous Catheter
• A tube placed in a large vein in the neck or chest
• Usually temporary
• Higher risk of infection



How to Protect Your Lifeline

Daily Access Care

✔ Check your access every day
• Feel the vibration (thrill)
• If the vibration stops, seek medical help immediately

✔ Keep the access clean

✔ Do NOT allow the following on your access arm:
• Blood pressure checks
• Blood extraction
• IV lines

✔ Avoid:
• Tight clothing or watches
• Sleeping on the access arm
• Carrying heavy bags on that arm



Infection Prevention 🦠

Infection is one of the most serious complications of dialysis access. You can help prevent it.

Before Dialysis

✔ Wash the access arm (AVF and AVG) with soap and water before every dialysis session

✔ Keep the skin clean and dry

✔ Do not scratch the access site



During Dialysis

✔ Make sure the staff cleans the skin with antiseptic before inserting needles

✔ Do not touch the needle sites during dialysis



After Dialysis

✔ Keep the dressing clean and dry

✔ Wash your hands before touching the access area



For Patients with Catheters

If you have a Central Venous Catheter:

✔ Keep the catheter dressing dry at all times

✔ Do not submerge in water (no swimming or soaking)

✔ Cover the catheter during bathing

✔ Do not open or handle the catheter tubing



Call Your Dialysis Team Immediately If You Notice

⚠ Redness
⚠ Swelling
⚠ Warmth or pain
⚠ Pus or discharge
⚠ Fever or chills
⚠ Bleeding
⚠ No vibration (thrill)



✅ Remember:
Your dialysis access allows life-saving treatment.

Protect it. Keep it clean. Check it every day.
Because your dialysis access is your lifeline.

Kung ikaw ay sumasailalim sa hemodialysis, ang dialysis access ang lugar kung saan lumalabas ang dugo mula sa iyong katawan, nililinis ng dialysis machine, at ibinabalik muli sa iyo.

Kung walang maayos na access, hindi maisasagawa ang dialysis.
Kaya ito ay tinatawag na iyong lifeline.



Mga Uri ng Dialysis Access

1. Arteriovenous Fistula (AV Fistula)
• Pinagdurugtong ang artery at vein sa pamamagitan ng operasyon
• Karaniwang inilalagay sa braso
• Ito ang pinakamainam at pinakaligtas para sa pangmatagalang dialysis

2. Arteriovenous Graft (AV Graft)
• Gumagamit ng synthetic na tubo para pagdugtungin ang artery at vein
• Ginagamit kapag hindi angkop ang ugat para sa fistula

3. Central Venous Catheter
• Isang tubo na inilalagay sa malaking ugat sa leeg o dibdib
• Karaniwang pansamantala lamang
• Mas mataas ang panganib ng impeksyon



Paano Protektahan ang Iyong Lifeline

Pang-araw-araw na Pangangalaga sa Access

✔ Suriin ang iyong access araw-araw
• Damhin kung may panginginig o vibration (thrill)
• Kung mawala ang vibration, magpatingin agad sa doktor o dialysis center

✔ Panatilihing malinis ang access

✔ Huwag payagan ang mga sumusunod sa access arm:
• Pagsukat ng blood pressure
• Pagkuha ng dugo
• IV line

✔ Iwasan ang:
• Masisikip na damit o relo
• Pagtulog sa access arm
• Pagbubuhat ng mabibigat na bagay gamit ang braso na may access



Pag-iwas sa Impeksyon 🦠

Ang impeksyon ay isa sa pinaka-seryosong komplikasyon ng dialysis access. Makakatulong ka upang maiwasan ito.

Bago ang Dialysis

✔ Hugasan ang access arm gamit ang sabon at tubig bago ang bawat dialysis session

✔ Panatilihing malinis at tuyo ang balat

✔ Huwag kamutin ang access site



Habang Nasa Dialysis

✔ Siguraduhing nililinis ng staff ang balat gamit ang antiseptic bago iturok ang mga karayom

✔ Huwag hawakan ang mga karayom o needle site habang nagda-dialysis



Pagkatapos ng Dialysis

✔ Panatilihing malinis at tuyo ang dressing

✔ Maghugas ng kamay bago hawakan ang access area



Para sa mga Pasyenteng may Catheter

Kung ikaw ay may Central Venous Catheter:

✔ Panatilihing tuyo ang dressing ng catheter sa lahat ng oras

✔ Huwag ilubog sa tubig (bawal mag-swimming o magbabad)

✔ Takpan ang catheter habang naliligo

✔ Huwag buksan o galawin ang catheter tubing



Tumawag Kaagad sa Dialysis Team Kung May Mapansin na:

⚠ Pamumula
⚠ Pamamaga
⚠ Mainit o masakit na pakiramdam
⚠ May nana o lumalabas na likido
⚠ Lagnat o panginginig
⚠ Pagdurugo
⚠ Walang vibration o thrill



✅ Tandaan:
Ang dialysis access ang nagbibigay daan sa buhay-na-nakapagliligtas na gamutan.

Protektahan ito. Panatilihing malinis. Suriin araw-araw.
Dahil ang iyong dialysis access ay iyong lifeline.
