As indicated in my previous post, here are the downloadable forms required by Manila Doctors Hospital only for those patients who have done prior online consultations and have arranged clinical appointments with me for further face-to-face or physical assessments in the hospital.

Please make sure to print and accomplish these forms, and bring them with you on the confirmed scheduled date/ time. Thank you!

Dear patients,

In response to the new normal that this pandemic has brought upon us, the physicians of Manila Doctors Hospital (MDH) are required to perform initial online consultations with patients before conducting face-to-face or physical assessments inside the hospital.

The first option for online consultations is via www.medifi.com. Kindly visit the site for proper instructions.

As an alternative to those without means for online payments, I have created this official page to allow direct online consultations with me through 3 simple steps:

1. Hit the Send Message button of this page for online consultations/ correspondence.

2. For patients that need further face-to-face or physical assessments in MDH, download the forms: Covid-19 Health Status Assessment Form & Schedule/Clearance Card. Forms will be available in my next post.

3. Accomplish forms and proceed to MDH UN Avenue entrance lobby on the appointed date/ time.

NOTE: Scheduling of appointments in MDH will be arranged and coursed through me during the online consultations/ correspondence.
