Case of Hepatic Flexure Ca where we did an Extended Right hemi with cmplete mesocolic excision + central vascular ligation using the Uncinate Process-First Approach. With the patient in steep T-burg, right-sided tilt, small bowels are swept to the RUQ. We start the dissection at the DJ flexure, moving at the dorsal aspect of the duodenum in a medial to lateral fashion. This dissection continues in the mesocolic plane to the cecum and the hepatic flexure. The patient is the repositioned reverse trend and we proceeded with the central vascular ligation by demonstrating the V-confluence of the Superior mesenteric vessels and the ileocolic pedicle. The meso is opened to meet the completed dorsal dissection. The SMV was the followed craniad ligating the ICV,ICA at its origin. The lesser sac is then entered and the mesogastrium separated from the mesocolon. Hepatic flexure takedown as per usual. Revisiting the vascular trunk, the middle colic vessels were also taken at its origin. The superior colic vein was also taken from the GCT.
