As part of the Philippine General Hospital's activities for the annual colorectal cancer awareness month, the Division of Gastroenterology will be conducting a screening colonoscopy drive in March 2024.
To be eligible for inclusion in the screening colonoscopy drive, each participant should meet ALL of the following criteria:
1) Be at least 50 years old*
2) Have NO symptoms
3) Be an active member of PhilHealth
*If with a first-degree relative (e.g. parents, siblings, children) ever diagnosed with colorectal cancer, the participant may avail of the screening colonoscopy service if he/she is at most 10 years younger than the age when the relative was diagnosed with colorectal cancer. For example, if you have a sibling who was diagnosed with colon cancer at age 52, you may already undergo screening colonoscopy even at 42 years old.

Aside from filling out this online form, please go to Central Endoscopy Unit (CENDU), 3rd floor, Philippine General Hospital for confirmation of participation, scheduling, and further instructions. Slots are limited, so confirmation of participation as soon as possible is encouraged.
Note: Accomplishing this online form DOES NOT GUARANTEE a slot in the screening colonoscopy drive. You may be contacted in case further clarifications are needed based on your responses to this online form, with the possibility of being excluded from the screening colonoscopy drive if you are eventually deemed not eligible. Please also ensure accuracy in answering this form, as your procedure may be deferred right on the scheduled date in the event inaccuracies are found
