Attended the Mass CPR and AED Launching of Ozamiz City today.
Proud and grateful to witness leadership with vision and heart. ❤️

Ozamiz City is now officially rolling out 68 AED units, to be distributed across all barangays — not just in crowded places, but even in the farthest communities.
A true commitment to saving lives.

As an interventional cardiologist, I cannot emphasize this enough:
In cardiac arrest, every second counts.
CPR + AED can double or even triple the chances of survival. 🫀⚡️

Congratulations to Mayor Sam Norman Fuentes ( and the previous Mayor and congressman of the city) and the entire city government for prioritizing cardiovascular health and empowering community responders.
And to my fellow cardiologists in Ozamiz — salamat kaayo sa inyong active engagement sa CPR-only training sa komunidad.

IMPORTANCE OF IMAGING & PHYSIOLOGY IN INTERVENTIONAL CARDIOLOGY

Interventional cardiology has evolved far beyond “see obstruction → put stent.”
Today, precision matters, and that precision comes from:

🫀 1. Imaging (IVUS / OCT)

Imaging allows us to see what angiography cannot.

Angiography = 2D silhouette only

It cannot show:
• Plaque composition
• Vessel size
• Calcium depth
• Stent expansion or malapposition
• True severity of diffuse disease
• CTO cap ambiguity

Imaging answers the questions angiography cannot:

✔ Is this lesion truly significant?
✔ How large is the vessel?
✔ Do we need aggressive predilatation?
✔ Is there deep calcium requiring atherectomy or IVL?
✔ Is my stent fully expanded and apposed?

Clinical Impact:
• Reduces stent thrombosis
• Reduces restenosis
• Reduces repeat PCI
• Reduces MACE
• Improves long-term outcomes
• Avoids unnecessary stenting

IVUS/OCT saves lives.
Imaging-guided PCI is now the gold standard, not a luxury.



2. Physiology (FFR / iFR / RFR)

Physiology tells us if a lesion is truly causing ischemia.

Why it matters:
• Angio can overestimate or underestimate severity.
• Borderline 40–70% stenoses often fool the eye.
• Visual assessment alone is wrong in up to 30–40% of cases.

Physiology clarifies:

✔ Is this lesion hemodynamically significant?
✔ Is stenting needed or can we manage medically?
✔ Which lesion is the culprit in multivessel disease?
✔ Is it epicardial or microvascular?

Clinical Impact:
• Avoids unnecessary stents
• Identifies lesions that truly need intervention
• Improves angina control
• Reduces cost and complications
• Guides complete vs selective revascularization

FFR/iFR turns guesswork into science.



🩺 IMAGING + PHYSIOLOGY = PRECISION PCI

They complement each other:

Physiology → Should I stent?

(Functional significance)

Imaging → How should I stent?

(Sizing, lesion preparation, optimization)

When used together:
• Fewer stents
• Better outcomes
• Personalized treatment
• Greater procedural success
• Safer, cleaner PCI

This is the future of interventional cardiology — evidence-based, imaging-guided, physiology-driven intervention.

Your presence emphasizes that regional initiatives become stronger when anchored on national support and shared expertise. Thank you for elevating and enriching our understanding of updated, evidence-based heart failure management.

To our colleagues, residents, fellows, nurses, medical students and allied health professionals, thank you for investing your time to learn, listen, and grow.
