Back 2 back cases of surgery with liver problems.
First case is gallstone in the setting of liver cirrhosis and
Second case is gallstone with perihepatitis. Note the violin string sign adhering from the liver to the peritoneum.
Kaya, alagaan natin ng mabuti ang atay natin.

Back to back cases of cholecystectomy from 7-10AM.
Bato free na sila at pwede na kumain ng kahit ano at kahit gaano. No more pain.

Hindi ko sana gagawin ang procedure dahil may ibang sakit ang pasyenteng ito. Kaso sakit na sakit na siya. So nagdecide na ako na gawin ang kaninang umaga. Ang nakuha ay isang napaka laking apdo na punong puno ng bato.
Nagpapahinga na siya ngayon sa kwarto niya at bukas makakauwi na siya.

A very peculiar case of a patient who had a ruptured gallbladder operated via laparoscopic cholecystectomy last Oct 2025. After a month, he complained of pain at the same area, the right upper abdomen.
Workups were done which included a CTSCAN of the abdomen. The CTScan showed a granuloma at the right re**us abdominis muscle.
I decided to operate via ultrasound guided exploration of the right re**us abdominis muscle.
Lo and behold, there was indeed a gallstone caught under the muscle just above the posterior re**us sheath!
Now I honestly do not know how it ended up there.

Ang unang bangon pagkaraan ng ilang oras pagkatapos ng kanyang gallbladder surgery.
Ganyan ang laparoscopy, bangon agad. Walang masyadong aray.

Ito ay isang xray. Ano ang nakikita natin dito? Yung nakabilog na dilaw ay isang . Kadalasan, hindi nakikita sa xray ang gallbladder. Pero sa kundisyon na ito, Kitang kita na anlaki ng gallbladder.
Bakit kaya? Ito’y tinatawag na Porcelain Gallbladder. Isang kundisyon na nagka calcium deposits na ang gallbladder sa Tagal ng May sakit o pamamaga nito. Lumaki at tumigas na ito.
Ano dapat gawin dito? Pag natuklasan na may porcelain gallbladder ang isang tao, dapat agad agad na tanggalin ito sa pamaraan ng laparoscopic surgery dahil ang porcelain gallbladder ay may malaking chance na maging cancer of the gallbladder.

Ang sabi ng ultrasound gallbladder . Hiningi ko sa pasyente ang film o picture. Sabi ko pag polyps hindi naman kelangan operahan. Pero pag tingin ko ng film Medyo malalaki ang sinasabi nilang polyps.
Ang pasyente naman, kinakailangang mag pa opera para makatrabaho.
So, inoperahan ko siya kanina, laparoscopic.
Ayun, hindi siya polyp. Gallstone siya.
