Ano pwede/bawal kainin pag may GS?
Sa madaling sagot, walang bawal! Lahat po pwede kainin.
Bakit?
Kasi ang katawan natin ay automatic. Basta malaman ng utak natin ma may laman ang tiyan natin, sasabihin niya si apdo na mag trabaho. Kahit ano pang pagkain ang kinain natin, magtratrabaho si apdo. At kapag may bato na nakaharang at hindi makalabas ang katas ng apdo, sasakit ang apdo.
Kung talagang pipilitin niyo kung ano dapat kainin para hindi mag trigger , eh ang sagot ay Huwag kumain!
Kasi pag hindi kayo kumain, naka relax ang apdo at hindi sasakit.
Ang problema ay yung bato, hindi pagkain.
Kaya ang solution ay dapat yung bato solusyonan.

A seemingly innocent ultrasound showing 2 gallstones. Patient had attacks a few months ago but no more pain recently but he decided to have surgery.
Hmm… Minsan nadadaanan sa kutob ito. Lahat ok pero parang sobrang nagmamadaling maoperahan ang pasyente.
Ayan na nga, halos bulok na ang kanyang apdo. Ang kapal na rin ng balot. At napakalaki na rin ang apdo, halos hindi na magkasya sa specimen bag.
Pero carry din naman, Medyo natagalan ang operasyon. Mga 3 hours lang naman.
Walang hiwa. 3 butas lang.

This patient messaged me here inquiring about laparoscopic cholecystectomy. When I saw her ultrasound, it says 2 stones. Patient told me that pain is getting worse in the last 6 months. So I scheduled here immediately.
Lo and behold, the cystic duct was super dilated with lots of stones inside. I couldn’t use a hemoloc to clip it but instead used a surgical loop tie.
When I opened up her gallbladder, the stones were tiny bit multiple and gallbladder is already hydropic. You can see the color of bile is clear already.

This senior was rushed to the ER because of severe abdominal pain last Valentine’s Day. Based on her ultrasound, there was a stone that is lodged in the neck of the gallbladder. Normally, she would be brought in for emergency surgery.
I did things a little bit different. I delayed surgery. I gave pain killers and asked her not to eat. After 2 days, I did the laparoscopic surgery. It was still inflammed but there are signs of regression of inflammation.
Surgery was done in 2 hours.
And I promised my patient that she will be eating a sumptuous meal on Chinese New Year! Which is in 5 hours time.

Sad news today:
3 yrs ago, may naoperahan akong acute choledocholithiasis with cholecystitis. Meaning, magang apdo na may bato at may nahulog din sa common bile duct. Nagawan ng ERCP at na laparoscopic cholecystectomy. Ngunit, may iniwan na stent sa common duct dahil marami rami bile sludge ang naiwan. Sinabihan namin ang pasyente na after 6 months, kailangang tanggalin yung stent.
Nakaraos na ang pasyente at hindi na nag follow up.
Kaninang hapon, tinawagan ako ng residente ko na may patient ako dati na nagpatingin sa charity. Ang nakita sa ultrasound ay yung stent na nasa common duct at punong puno ng bato.
Nag decide narin ang pasyente na mag pa open surgery.
Ano ngayon nasa isip ko?
Ang stent ay isang “foreign object” dapat tinatanggal yan within a specified time. Kung hindi tinanggal, pwede ito maging sanhi ng pagkabuo ng bagong bato. Katulad ng nangyari sa pasyente.
Tinignan ko ang record ng pasyente at naalala ko na ang kaso niya ay pinagtyagaan kong gawin na lap, umabot ako ng 4 na Oras bago ko natapos. Sobrang nakakahinayang ng dahil sa isang stent na hindi tinanggal, maooperahan siya muli.

We use this to biopsy solid tumors of the breast, prostate and other tumors of the neck. It is G18 and avoids opening up of the skin and violating the tumor.
Patient can go home immediately after the procedure.
