🚑 From the Field to the Floor: Bridging EMS and Nursing with SBAR

In EMS, our job doesn’t stop at the hospital doors—our handoff is the bridge between prehospital care and inpatient care. To make that bridge stronger, I started using a printed handoff sheet. It’s simple. It’s fast. And it mirrors the language that nurses already use: SBAR.

Here’s how I structure my EMS reports with SBAR in mind:

🟠 S – Situation:
Chief complaint + what brought us here
Example: "Patient is a 64-year-old male with chest pain and SOB that began 30 minutes ago at rest."

🟡 B – Background:
Scene details, past medical history, and medications
Example: “Found sitting upright in bed, hx of HTN and CAD, took nitro x1 prior to arrival.”

🔵 A – Assessment:
Pertinent findings and trends—vitals, ECG, exam, interventions
Example: “Vitals stable with sinus tach, 12-lead showed ST elevation in II, III, aVF. Nitro and O₂ administered with symptom relief.”

🟢 R – Recommendation:
What I think is most important moving forward
Example: “Request immediate cardiac workup. Concern for inferior STEMI based on 12-lead and presentation.”

By aligning EMS handoffs with the SBAR framework, we’re not just sharing information—we’re speaking the same clinical language.

✅ It’s safer for the patient
✅ It builds trust across disciplines

We don’t need a policy change to improve communication—we just need shared standards and mutual respect. Whether you're in scrubs or a uniform, SBAR works—and using it across care settings makes all of us stronger.

The difference between an ambulance and a PTV (Patient Transport Vehicle) lies in their purpose, equipment, staffing, and clinical capability.

🚑 Ambulance
Purpose:

-Used for emergency or urgent medical situations.

-Designed to provide pre-hospital care and rapid transport to medical facilities.

Staffing:

-Staffed by paramedics, EMTs, or nurses trained in advanced life support (ALS) or basic life support (BLS).

Equipment:

-Equipped with emergency medical equipment (defibrillator, oxygen, cardiac monitor, medications, etc.).

-Can manage critically ill or injured patients during transport.

Use Cases:

-911 emergency calls

-Critical interfacility transfers

-Accidents, strokes, cardiac arrests, etc.

🚐 PTV (Patient Transport Vehicle)
Purpose:

-Used for non-emergency, low-acuity patient transport.

-Designed to safely move stable patients between healthcare locations or to/from their homes.

Staffing:

-Staffed by patient transport officers, drivers, or basic care assistants.

-Usually non-clinical staff or only BLS-trained personnel.

Equipment:

-Basic equipment for comfort and safety (wheelchair access, stretcher, first aid kit).

-No advanced life-saving tools or medications.

Use Cases:

-Discharges to home

-Routine medical appointments

-Scheduled hospital transfers for stable patients

📝 Summary:
Ambulance = Emergency, equipped, medical staff onboard

PTV = Non-emergency, stable patient, limited equipment
