2️⃣ Work Experience Sheet (can be downloaded at csc.gov.ph)
3️⃣ Certificate of Residency
4️⃣ 2pcs 2x2 picture
5️⃣ 2 copies of NBI clearance
6️⃣ 2 copies of PRC Board Rating (Certified True Copy by the issuing Agency)
7️⃣ 2 copies of PRC ID (Certified True Copy by the issuing Agency)
8️⃣2 copies of PRC Certificate of Good Standing (CTC of the issuing Agency)
9️⃣ Medical School Diploma/ Transcript of Records (CTC of the issuing School/University)
🔟 2 Copies of PSA Birth Certificate
1️⃣1️⃣ 2 Copies of PSA Marriage Certificate
1️⃣2️⃣ Photocopy Certificate of Employment / Service Record with Summary of Duties relevant to the position being applied to (Authenticated by the issuing Agency)

The Bicol Medical Center proudly welcomes Senator Imee Marcos. She conducted an ocular visit to the Cardiac Center and expressed her heartfelt gratitude to the dedicated medical frontline workers of the BMC-Cardiac Center.

Moreover, she acknowledges the steadfast dedication to ensuring quality and best cardiovascular care to Bicolanos. Sen. Imee Marcos visited the four (4) pediatric patients who were part of the 2nd Cardiovascular Surgical Mission last March 15, 2024.

Last March 15 , 2024 four patients from remote areas underwent closure of their congenital heart defects patent ductus arteriousus tru device closure done by Dr. Eloisa C. Barrion, MD together with anesthesiologist Dr Sarah DC. Angeles, MD and with the help of Philippine Heart Center Pediatric Interventionalists Dr Jean Antonio G.Villareal, MD and Dr Babie Catherine R.Causapin, MD.

She was also ushered by the
pediatric patients who successfully underwent surgical and interventional procedures during the very 1st Pediatric Surgical and Interventional Mission last March 2023. These patients were the beneficiaries of her " Philippine Heart Warriors".

Sen. Imee Marcos' visit was followed by a press conference together with Dr Juliet Balderas, MD the Department Manager of Pediatric Cardiology Philippine Heart Center, Dr. Mary Jane Uy, MD Chief of Medical Professional Staff in behalf of Dr Ronnie Gregorio Gigantone III, MD BMC Medical Center Chief and Cardiac Center Head Dr Eloisa Victoria C. Barrion, MD.

The office of Sen. Imee Marcos pledged to offer additional support for patients undergoing Open and Closed Heart Procedures. A portion of the expenses will be supported through the allocated budget of the DOH Medical Assistance to Indigent Patients (MAIP).
