📂 REQUIREMENTS FOR THE APPLICATION:

👉Application letter addressed to the Medical Center Chief:

1️⃣ 2 copies of Fully Accomplished and Notarized Personal Data Sheet with recent passport-sized picture (CS Form No. 212 Revised 2017)
2️⃣ Work Experience Sheet (can be downloaded at csc.gov.ph)
3️⃣ Certificate of Residency
4️⃣ 2pcs 2x2 picture
5️⃣ 2 copies of NBI clearance
6️⃣ 2 copies of PRC Board Rating (Certified True Copy by the issuing Agency)
7️⃣ 2 copies of PRC ID (Certified True Copy by the issuing Agency)
8️⃣2 copies of PRC Certificate of Good Standing (CTC of the issuing Agency)
9️⃣ Medical School Diploma/ Transcript of Records (CTC of the issuing School/University)
🔟 2 Copies of PSA Birth Certificate
1️⃣1️⃣ 2 Copies of PSA Marriage Certificate
1️⃣2️⃣ Photocopy Certificate of Employment / Service Record with Summary of Duties relevant to the position being applied to (Authenticated by the issuing Agency)
