PWD APPLICATION PROCESS
If you're in Quezon City and need to apply for a Persons with Disability (PWD) ID, you have two convenient options: online registration or visiting a local satellite office.
🖥️ Option 1: Online Application via QC E-Services
1. Register an Account
Visit the QC E-Services portal and create an account using your personal email address.
2. Complete the Application
Log in and select the “QCitizen ID eApplication” icon. Fill out your personal information and ensure you indicate “Yes” for the PWD sector.
3. Upload Required Documents
Prepare digital copies of the following:
Proof of Disability:
Apparent Disabilities: A full-body photo clearly showing the disability.
Non-Apparent Disabilities: A medical certificate and a certificate of disability from a licensed specialist.
Proof of Residency: Any government-issued ID or an original Barangay Certificate.
ID Photo: A 2×2 photo with a white background.
Signature or Thumbmark
4. Submit and Await Approval
After submission, your application will be reviewed. Upon approval, you'll receive a notification, and your digital QCitizen ID will be available through the QCitizen app.
🏢 Option 2: Walk-In Application at PDAO Satellite Offices
For in-person applications, you can visit any of the following Quezon City PDAO satellite offices:
District I: #176 E. Beltran St., Brgy. Katipunan
District II: Commonwealth Barangay Hall, Commonwealth Ave.
District III: #25 Calderon St., Brgy. Marilag, Project 4
District IV: Archival Center, #65 Scout Reyes St., Brgy. Paligsahan
District V: Novaliches District Center, Jordan Plains Subdivision, Novaliches Proper
District VI: Sanville Covered Court, Cenacle Rd., Sanville Subdivision, Brgy. Culiat
Requirements for Walk-In Application:

Processing Time: Typically, the ID is processed within 2–3 days. For non-apparent disabilities, additional validation may be required.
ℹ️ Additional Information
Eligibility: Residents of Quezon City with a long-term physical, mental, intellectual, or sensory impairment that substantially limits one or more major life activities.
Validity: The PWD ID is valid for five (5) years and is recognized nationwide.
