"My baby started letting go of my breast naturally between feeds – before, she'd fall asleep and re-latch when I tried to unlatch her."
This is my 3 month-old patient referred by a neonatologist friend due to a possible lip-tie.
She was admitted at 5 days of life due to dehydration and was given donor milk. Mom started expressing milk to boost her supply on top of direct breastfeeding. Breastfeeding usually took 20 -45 minutes every 1.5-3 hours. There was breastfeeding pain and baby would fall on and off on the right breast. There were choking and spillage during feeding. Baby had head turning preference to the right and breast preference on the left and heard snoring. On physical examination she has open mouth posture, blisters on the upper lip, with lip-tie and tongue-tie type 4. She had a sensitive gag reflex . During breastfeeding, there's a shallow latch with clicking sounds and spillage of milk more on the right breast. We released her oral ties that day and advised them to do oral exercises and stretching.
A week after frenotomy, mom reported that baby was not able to latch on the breast directly for 24 hours but accepted syringe feeding but resumed breastfeeding after that pain-free. Breastfeeding would last 15-20 minutes every 2 to 3 hours and baby would naturally let go of the breast by herself. There's no more choking episodes, minimal spillage, no more breast preference and decreased clicking sounds during feeding and less snoring. The wound from frenotomy had good healing with no reattachment.
One month after frenotomy, there's no more snoring and spillage rarely clicking sounds. Tongue healed well without reattachment. Praise God 🙏
A number of moms of babies with oral ties would report their babies to fall asleep in between feedings but would not let go when they try to remove their breast. It's actually tiresome to feed with oral ties that they preserve their energy by resting in between feedings but would not let go because they are still not full. After frenotomy, baby is able to latch effectively in a short time without getting tired so they can finally let go of the breast by herself.
