๐ DOH Rollout for Basic and Intermediate Training for Palliative and Hospice Care at ActivCare
It was truly a meaningful day for us at ActivCare as we welcomed representatives from the Department of Health (DOH) โ doctors, nurses, dieticians, and social workers โ for the rollout training on Basic and Intermediate Palliative and Hospice Care. ๐
The visit was more than just a training session; it was an eye-opening experience that gave healthcare professionals a real look into how community-based palliative care services work.
Guided by Dra. Rhea Pobre-Jayma, Ricky Rosales, Senior Nurse Case Manager Vergel Gutana, and Training Specialist Rose Nuez, the participants were able to immerse themselves in the process of providing compassionate care outside the hospital setting.
Through this hands-on program, they:

Explored the Interdisciplinary Team (IDT) approach, learning the vital roles of doctors, nurses, pharmacists, and dieticians in delivering holistic care.

Understood the referral flow from hospitals or homes, ensuring smooth coordination for patients and families.

Experienced how to conduct initial assessments, discharge planning, and collaboration with hospital staff.

Learned how to design and implement personalized CARE PLANS to be carried out at home by Palliative Care Nurses (PCN) and Palliative Home Assistants (PHA).
This collaboration between DOH and ActivCare is more than just training โ itโs about equipping healthcare professionals with both knowledge and heart. Together, we aim to raise the standard of palliative and hospice care, ensuring that patients with life-limiting illnesses and their families receive care that is not only professional, but also deeply compassionate.
