Mag-register dito upang mailista sa pila ng The Medical City Developmental Assessment para sa mga pasyente na INDIGENT.

Ito po ay bukas para sa PEDIA patients (0-18 years old lamang).

DISCOUNTED RATES po ito para sa mga pasyente na maaaprubahan ng social service na magiging qualified.

Maari lang po i-handa ang mga sumusunod:
1. CERTIFICATE of INDIGENCY (galing sa inyong barangay) na nakapangangalan sa MAGULANG/TAGAPANGALAGA ng bata at para sa MEDICAL ASSISTANCE.
2. Valid ID ng magulang/tagapangalaga
3. PAYSLIP or PROOF OF INCOME ng mga MAGULANG (PAREHAS NA MAGULANG) /TAGAPANGALAGA (PAREHAS NA MAGULANG) ng bata na sakop ang nakaraang 3 buwan.
Kung WALANG PAYSLIP:
PROOF OF INCOME (kasama ang part time at business)
Certificate of employment (local dapat naka lagay ang monthly salary)
Contract of employment (seaman, ofw and etc).
Kung ang pinagmumulan ng pang-araw-araw na gastusin ay sa mga kamag-anak, pakipost po ang kanilang payslip/proof of income
4. Philhealth ID ng magulang/tagapangalaga (kung meron)
5. PWD ID ng bata (kung meron) OR school ID ng bata OR Birth Certificate ng bata

Pakiantay po na icontact kayo ng social worker ng TMC through VIBER base sa number na inyong niregister. Ang mga dokumentong ipapasa ninyo ang gagamitin para sa interview with social worker upang malaman kung qualified kayo para sa serbisyong ito.

Hinihiling na ISANG beses lamang mag-register.

Ang mga mag-upload ng peke or maling dokumento ay maaring matanggal sa listahan.

Ang tamang proseso:
1. Kumpletuhin ang mga hinihinging dokumento.
2. Mag register ng TAMANG detalye at iupload ang mga TAMANG dokumento.
3. Antayin na icontact ng social worker thru VIBER na nilagay sa registration apra sa ONLINE interview. Mahaba ang pila ng for interview ng social worker.
4. Bibigyan kayo ng schedule sa for developmental assessment kapag nainterview na kayo ng social worker.
