Many providers claim that their imitation powders are more affordable and work fine with EMS instruments. In fact, the opposite is true. A copy is never a match on the original. These powders can easily perforate EMS AIRFLOW handpieces and damage the interior of the EMS AIRFLOW devices. Can you imagine the harm that these powders can do to your patients’ teeth? EMS strongly recommends that dental practitioners only treat their patients with compatible powders. Otherwise, their warranty may become void and expensive repairs may be necessary. EMS emphatically warns against the use of non-approved powders from imitators, copycats or even counterfeiters. Beware! They do not comply with our strict quality criteria at all. 38 years old female patient complained from a localized gingival mass that was excised 8 years ago and recurred again 2 months ago .......it was sessile reactive lesion related to interdental papilla between upper 2 central incisors which was lacerated and inflamed.......

Treatment plan began with phase I therapy including scaling and deep gingival curettage beneath the lesion over 3 sessions......

After first session of scaling there was a marked decrease and shrinkage in the size of the lesion and so in the 2nd and 3rd session till it nearly completely resolved and this is common in inflamed reactive lesions which are usually responsive to deep gingival curettage ....so in many of cases they didn't require surgical excision unlike fibrous non inflammatory reactive lesions which are non responsive to curettage and need excisional biopsy .....

This manner usually occurs with pyogenic granuloma....

Follow up is needed every 3 months to monitor any recurrence.....so it is mandatory to instruct the patient to follow strict oral hygiene measures as these lesions can easily recur if the gingiva became inflamed again due to any source of irritation

Tips that I have learned over time to reduce chances of instrument fracture.

1 - Straight line access to avoid unnecessary pressure on the instrument.

2 - Always start with pathfinder files and then gradually increase the size.

3- Almost always use EDTA (root canal lubricant) the first time you are cleaning the canal.

4- File bend test on a flat surface before using.

5- Never use old files, keep checking the files with a magnifying glass or macro camera for any loose flutes/corrosion etc.

6- Always use high-quality original files.

Lingual Nerve.
The proximity of the lingual nerve to the mandibular third molar region is a concern when performing flap surgery. In a study by Chan et al 2010, the vertical distance was 9.6, 13, and 14.8 mm at the second molar, first molar and second premolar, respectively. About 75% of the time, the nerve turned toward the tongue at the first and second molar region.

Other studies have shown that on average, the nerve is located 3mm apical to the osseous crest and 2mm horizontally from the lingual cortical plate in the third molar area. 22% of the time, the lingual nerve can actually contact the lingual cortical plate.
