What Is Optimum Care Program?

The CareHealth Plus Optimum Care Program is a 10-year program with a paying period of only 5 years that provides comprehensive hospitalization services, outpatient care and life insurance benefits to its Members.

Aside from the 10-year coverage, Optimum Care medical benefits are extended on the 11th year until the 15th year with Maximum Benefit Limit (MBL) that continues to increase yearly by 5% or 8%. Medical benefits may be assigned to immediate family members up to the Second (2nd) degree of consanguinity/affinity on its extended years.

Who Can Avail The Optimum Care Program?

To avail this program, prospective Members must be from age 7 years old to 65 years old.

What Are The Options For Optimum Care Program?

Members can choose among the two (2) plans that this program offers:

Five (5) Units with a minimum annual premium of P46, 780
Ten (10) Units with a minimum annual premium of P88, 263

What is Quantum Gold Program?

Quantum Gold Program offers a health care coverage payable for only 5 years. Members will then continue to enjoy the benefits of this program for another two (2) years which are, the 6th & 7th year of coverage.

Who Can Avail The Quantum Gold Program?

To avail this program, prospective Members must be from age 7 years old to 65 years old.

What Are The Options For Quantum Gold Program?

Members can choose among the two (2) plans that this program offers:

Five (5) Units with a minimum annual premium of P24, 705
Ten (10) Units with a minimum annual premium of P45, 457

Value Plus Program

The Value Plus Program is a two (2) plus one (1) CareHealth program for any individual. It has a paying period of 2 years with an additional 1 year free coverage, totaling for 3 years of coverage. It has a unique feature of health care benefits plus financial assistance in case of death or disability.

Who Can Avail The Value Plus Program?

To avail this program, prospective Members must be from age 7 years old to 70 years old.

What Are The Options For Value Plus Program?

Members can choose among the three (3) plans that this program offers:
