1. Please bring the following at the vaccination site:
-ID of the person receiving the vaccine
-An authorization letter from the parent, if only a guardian is accompanying a minor/child.
-A pen for each family
-Digital proof of payment
2. Wearing of face mask is optional but encouraged for health and safety purposes.
3. Individuals with moderate to high fever and/or serious illnesses shall not be allowed to undergo vaccination. Basic health assessment and screening will be done for free on the day of vaccination.
