Extracorporeal shock wave lithotripsy (ESWL) is one of the available treatment options for urinary tract stones. It is non-invasive, involving no incision and minimal anesthesia. During ESWL, the stone is located using a combination of x-ray fluoroscopy and ultrasound, then a special machine directs precisely focused high-energy pressure waves to shatter the stone without causing harm to surrounding tissue. According to European Association of Urology (EAU) guidelines, ESWL is recommended for urinary tract stones up to 1.0 cm in size, in any location, and may also be used for larger stones in selected cases.

PCNL, or percutaneous nephrolithotripsy/nephrolithotomy, is a minimally-invasive treatment for kidney stones. It has supplanted the role of traditional open surgery as treatment of choice for stones larger than 2 centimeters. PCNL is also a recommended treatment option for some smaller stones otherwise unsuitable for non-invasive treatment due to hardness/density, multiplicity, or unfavorable location within the kidney.

In simple terms, PCNL involves the establishment of a temporary tract through a small skin incision, into the interior space of the kidney. A rigid endoscope and a variety of instruments are used through this tract in order to remove the stones. In the case presented here, a hollow ultrasonic probe is used to pulverize the kidney stones with high-frequency vibrations and also simultaneously extract the dust and fragments with suction. Compared with traditional open surgery, PCNL provides similarly excellent success rates for complete stone removal, but with less pain and discomfort, less blood loss, and faster recovery after the procedure.

Radical orchiectomy is an essential early step in the management of a suspicious testicular mass, after possible non-neoplastic causes such as trauma or infection have been considered and proven unlikely. It contributes in multiple ways, including: 1) definitive histopathologic diagnosis of cancer, 2) tumor staging, and 3) treatment. In organ-confined (Stage I) testicular cancer, radical orchiectomy is often sufficient to achieve lifelong cure, without need for any other treatment such as radiation or chemotherapy. If you have noticed an unusual lump or enlargement in one testicle, it is best to seek consult with a certified urologist as soon as possible in order to evaluate for the possibility of testicular cancer.

My practice encompasses a wide variety of urologic conditions, including but not limited to: urinary tract stones, obstructions, infections and trauma; prostate disease; infertility; cancers of the genito-urinary organs (kidneys, bladder, prostate, testicles and p***s) and adrenal glands. I am trained and certified in minimally invasive urologic techniques, including percutaneous nephrolithotomy/nephrolithotripsy, retrograde intrarenal surgery, laser enucleation of the prostate, and upper urinary tract laparoscopic surgery.
