The major concern for this patient’s nose was the length of the nose. We decided to elongate the nose while subtly improving the height and projection.
Lengthening the nose in rhinoplasty is a complex maneuver that involves increasing the vertical distance from the radix to the tip while maintaining structural stability and a natural appearance. This is commonly performed in patients with a short nose, over-rotated tip, contracted nose, or previous rhinoplasty with scar contracture. The procedure often requires strong structural support using septal cartilage, ear cartilage, rib cartilage, or synthetic support materials depending on the patient’s anatomy and surgical history. Techniques such as septal extension grafts, columellar struts, tip grafting, and release of scar contracture may be used to reposition and support the nasal tip in a more projected and lengthened position.
The goal is not simply to make the nose longer, but to create better facial harmony, smoother nasal lines, improved tip support, and a balanced relationship between the bridge, tip, and upper lip. Healing also plays a major role, as swelling and tissue relaxation over time contribute significantly to the final definition and overall refinement.
