Oral Cutaenous Fistula Secondary to Failed Root Canal Treatment- performed at the Mall by a MALL DENTIST(well-known along the area)

Patient came in with a facial manifestation of Fistula Originating from the proximity of the Apex of Upper Right Maxillary Canine. Upon consultation, the was a 4-unit(15,14,13,12) bridge that failed and broke within the area where the fistula was suspected to have originated from. When the patient removed the bridge effortlessly, the teeth underneath were severely carious.

Initial periapical xrays were taken and there we found out that the concerned teeth had undergone ROOT CANAL TREATMENT already but werent done correctly and properly. All the teeth were obturated short by about 4-5mm almost done just past the middle third of the roots.

Luckily, there was enough bone and ligaments to hold the teeth and enough tooth structres left making it possible to just redo the ROOT CANAL TREATMENT and perform GINGIVECTOMY (to gain more ferrule) and post &core to augment the crowns for future refabrication of another Porcelain Fused to Metal(PFM) Bridge. These were all done under a month, just 3 weeks to be exact.

Today we were able to cement the newly fabricated bridge with well covered margins and the patient was instructed to still come back since he resides now in Italy for further treatment of the anterior and left side. Thank you for your patience sir and take care. Another smile restored.

Orthodontic Treatment(Braces) to Laser Whitening

When should and can you whiten your teeth? During or After Braces?
:You should only whiten your teeth AFTER your orthodontic treatment. As the chemical agents/whitening solution cannot pe*****te the brackets leaving the site of the brackets yellow.

Do we recommend whitening toothpastes during your orthodontic treatment?
:No because it may cause uneven shade of the tooth. Again it cannot pe*****te the area under the brackets.

Oral Prophylaxis(Cleaning)

Smoking is one of the major risk factors for periodontal (gum) disease. The chemicals in to***co reduce blood flow to the gums, limiting oxygen and nutrients needed for healing. Smoking also weakens the immune system, making it harder for the body to fight bacteria that cause gum infection. Canted Smile Correction

This patient had undergone a 3 yr ortho treatment from another clinic, molar relationship was left class 2, significant overjet of about 5mm and the most obvious concern was the canted(slanted) occlusal of both the maxillary and mandibular arch of about 4-5 degrees.

TADs was used to address the canting and this is just the treatment from April- September 2025. Further corrections will still be made and take note that this patient is using a SELF LIGATING type of brackets as he is an OFW, working as a teacher in Malaysia.

Ps:the hole in between the premolars was from the tads. It became loose so we transfered it in the molar area region.

Note: upper right quadrant is already over corrected as relapse is expected while extruding the lower right quadrant.
