Facial cellulitis invading the maxillary and submandibular fascial space - dental origin.

This patient suffered from sudden facial swelling, limited and restricted mouth opening(less than 5mm) and pain. Orthopantogram(OPG) revealed an impacted upper left wisdom tooth and a severely carious lower left first molar.

On his 3rd day of using broad spectrum antibiotics, significant relief was felt and observed and we were able to fully assess the suspected teeth/teeth involved. Operculitis on the upper left wisdom and gingival swelling on the buccal area of the lower left first molar was still evident.

Lesson: wag hayaan ung mga impacted na ngipin, lalo na ung mga bulok dahil maari talaga mangyari itong mga bagay na ito. Worst case scenario, magkaroon ng drainage sa labas ng muka that would definitely leave a scar and indentation.

Schwarz Expander Appliance(Slow Expansion Appliance)

Jaw expansion was initiated first prior to orthodontic treatment to treat the narrow arch of my First Twin patient. As you can see on the upper photo(Feb 9, 2025), this was his presenting case, narrow lower arch and crowding. Saggital expansion is needed to correct the bite and also the airway of my growing patient.

After 3 months(May 27,2025) , excessive deep bite was also somehow corrected due to the expansion from the appliance. Both the anterior and posterior area were sagittally expanded sufficiently so we proceeded already with a fixed appliance treatment using brackets. Further alignment, levelling and bite correction would be corrected after his orthodontic treatment phase.

So excited to finish my twin's case. Goodluck on your first orthodontic(braces) treatment month. Literally our tiis ganda case but worth it.

Multidisciplinary Case - Holy Week Special

This patient sought a consult and treatment for her failing bridge and pain. Upon checking, she had a PLASTIC BRIDGE used as a "permanent restoration" from another clinic.

After removing the bridge, as expected by Doc HB, the teeth under the bridge had a recurrent caries and the tooth preparation by the previous dentist was gross and structurally/anatomically incorrect. Margins were corrugated, some of the proximals were overprepped and it was as if done by an individual who has little to no knowledge about the fundamentals of Fixed Restorations.

Her condition that time has left her with only two options, to either have the damaged teeth extracted and removable denture can be fabricated or second save what could be saved and wait for her deteriorating condition to be controlled and subside and proceed with another Fixed Restoration.

She and her family chose the longer yet periodontically forgiving procedure, long term- which was the multidisciplinary treatment

Treatments Done Were:
1.) Removals of her failing bridge.
2.) Save and restore what's left through Root Canal Treatment(of 3 Anterior Teeth).
3.) Post And Core to correct the Tooth Preparations of the future Bridge.
4.) Periodontal Treatment of 2 Months- controlling the mobility, swelling of the gingiva, bleeding and plaque indices and the pseudo pocket made by the ill fitting bridge.
5.) Finally, the Esthetic Part which was the Prosthodontic Management- Fixed Bridge using Porcelain Fused to Metal.

Note: Lesson, always find a credible dentist/clinic. And dont settle for any Plastic Restoration for Permanent Treatment or Long Term use.

Direct Composite Veneers using Zirconia Reinforced Composite Resin.

This patient had a dull, off shade, big and fractured restoration on her Central Incisors.
She wanted to have it revised using a more sturdy Composite Material so we opted for a Zirconia Reinforced Resin to perform this conservative yet AESTHETIC procedure.

Her Central Incisors now are symmetrical, natural-looking and well polished.

Anterior Biteplane Appliance

This is to correct the Accentuated Curve of Spee by flattening the mandibular plane (lower teeth) thus treating the Severe Deep Bite of our patient.

Extrusion of posterior teeth leading to increased vertical dimension was the mechanics of choice on this particular patient to address also the TMJ issues that he is also suffering from; clicking and mandibular lateral displacement(upon opening).

Ps: Patient wore a mandibular splint for 6 months as his stage 1 pre ortho treatment.

Direct Composite Veneer(DCV) on Peg Lateral Incisor using Zirconia Reinforced Forma Composite and Shofu Beautifil Built in LAYERS.

DCV was done after proper establishment of midline, canine and molar relationship. This procedure was vital prior to the finishing of our patient's orthodontic treatment to prevent his case from having asymmetrical space distribution which could lead to off midline and improper canine and molar relationship establishment.

P.S. Dont settle for a monochromatic , unrealistic and mentos looking Veneers!
