TERMS AND CONDITIONS:
1. The Use of the Vault includes its maintenance and accomodation of up to TWO (2) URNS.
2. The Cremation Service will include FREE cremation exclusively for the Contract Holder.
3. Memorial Services, other than the Contract Holder, may have charges.
4. These Prices are subject to change without prior notice.
5. Credit Life Insurance Benefit provided by Sun Life Financial is given FREE to Installment Buyers subject to the Insurability Clause.
6. Post-dated checks should be issued on all purchases made on Installment basis.
7. Amortizations have an interest rate of 12% per annum based on Annual Mode.
8. Please issue post-dated checks in the name of The Everliving Company, Incorporated.
9. A Reservation fee of P5,000 is acceptable. Purchase should be completed within 7 days. Fee is not refundable.
10. Full payment is required if the Vault and Services will be used.
11. Fees apply to assignments, amendments, transfers, etc.
