• None of the Pacific Cross authorized providers require payment in advance to use covered healthcare benefits.
• Hospitalization benefits for patients, including room and board, use of the operating room, and intensive care.
• Chemotherapy, radiation, dialysis, and solitary confinement, among others.
• Outpatient benefits, which include minor surgery, lab tests, and unlimited medical consultations.

• Pre-existing conditions may be covered in accordance with the Schedule of Benefits and writing policies of Pacific Cross. Depending on the number of Principal Members at the Agreement's implementation date, individual health declarations may be exempted.
