IMPORTANT ADVISORY

Please be informed that effective 27 MARCH 2026, our office shall adopt a four-day onsite workweek, with FRIDAYs designated as work-from-home (WFH) days.
Members may file their claims and other concerns through either of the following:
• Submission via official email, (chsiliquidation@chs.com.ph) especially for Members outside Metro Manila; or
• Personal filing at the CHSI Head Office during regular business hours (Monday to Thursday - 9:00AM to 4:00 PM) only.
Consistent with government policy, this measure is implemented to reduce building energy consumption and transportation fuel use in light of ongoing global conditions.

Pursuant to the directives of the Insurance Commission (IC), please be advised that all Members whose names are not yet reflected in the CHSI Portal (https:/chs.com.ph/chsicp/) must file their claims on or before 02 MARCH 2026 (Monday).

Per the Notice to the Public released by the Insurance Commission, “claims filed after 02 MARCH 2026, shall be BARRED from the normal liquidation proceedings….” Instead, late-filed claims shall be referred to the Company for possible reconsideration during its dissolution and winding-up proceedings, subject to applicable laws and regulations.

Members may file their claims through either of the following:
• Submission via official email (chsiliquidation@chs.com.ph), or
• Personal filing at the CHSI Head Office during regular business hours (Monday to Friday, 9:00AM to 4:00 PM)

Concerned Members are strongly encouraged to file their claims within the prescribed period to ensure proper evaluation and inclusion in the liquidation proceedings, in strict compliance with the Insurance Commission’s directives.

Head Office Holiday Advisory

Please be advised that our Head Office will be closed on December 29, 2025 (Monday) and January 2, 2025 (Friday) in observance of the New Year, in line with the government's work suspension advisory - Memorandum Circular no. 111.

Advisory: No Fees for Filing Claims
As of 22 August 2025
It has come to our attention that there are certain persons offering their services for a fee to file your benefits claims with Caritas Health Shield.
PLEASE BE INFORMED THAT CARITAS HEALTH SHIELD:
• DOES NOT CHARGE fees for the filing and processing of claims;
• DID NOT AUTHORIZE any person to collect fees on its behalf for the filing or processing of claims; and
• FILING OF CLAIM(S) with Caritas Health Shield is FREE OF CHARGE.
MOST IMPORTANTLY, please refrain from divulging any personal information to persons claiming to be Caritas Health Shield representatives.
If you encounter such unscrupulous persons, please report the matter immediately to the Customer Relations Department ASAP.

Effective December 4, 2023, you can now email your PTV requests! PTV filing made easy!

Filing of Plan Termination Value (PTV) via email

1. Go to www.caritashealthshield.com.ph
2. Click Contact Us
3. Go to Select Subject and choose Plan Termination Value (PTV)
4. Download and Print Application Form for PTV (click the link)
5. Fill out the form.
6. Prepare a letter of request of your intention to file for PTV.
7. Email the following together with the Application Form for PTV and your letter in PDF or JPEG format at ptvclaim@chs.com.ph

• Photocopy of Policy Contract
• Photocopy of Membership ID Card or Green Card
• Photocopy of Certificate/Notice of Full Payment (for Fully Paid plans only)
• Photocopy of two (2) valid government IDs with three (3) fresh specimen signature

8. All successful PTV requests sent through email shall be duly acknowledged.
