To end the year on a high note, let me present to you this case!

ANOTHER DAY, ANOTHER FINISHED CASE!

Looks easy right? It' s just spaces right?

Just recently I saw a case almost similar to this but easier. The patient was a teenager and not suffering from PERIODONTAL DISEASE. And according to the Patient, his Dentist used Powerchain all throughout. As a result of that approach, the Upper and Lower Teeth tilted towards the Palate and Tongue. 😔

Yes you read that word "PERIODONTAL" , this case of mine was an Adult and already suffering from Generalized Periodontal Disease, which means there was Bone Loss and Most Teeth were Loose or with Mobility, that was what made this Case CHALLENGING!

Look how SEVERE WERE THE CLASS 2 CANINE RELATIONSHIPS!

The Finish isn' t Perfect but Highly Acceptable given the Condition of the Supporting Structures of the Teeth!

So there you go, MIDLINES COINCIDING, CLASS 1 CANINE RELATIONSHIP, GOOD CONTACT POINTS! NO TOOTH LOSS!

This patient came all the way from Infanta, Quezon!

Had 2 previous consultations, but the Patient' s Mother claimed those Dentists were too eager to operate on his Son and offered very little explanation for the Surgical Procedure and its Complications.

As a Trained (and Certified) Dentist in Hospital Dentistry and Oral Surgery, I would say we have developed a Clinical Eye different from those who had short courses.

I explained the Anatomy of the Impacted Teeth and all the Structures surrounding them. And discussed in detail the Intra and Post Op Complications such as Arterial Bleeding and Neuropathies to name a few.

As you can see the anatomy of the roots of both Impacted 3rd Molars are a bit hazy and not as clear. This gave me a suspicion that they might have a Varied Anatomy in terms of the Number of Roots and Curvature. They are Vertically positioned and the one on the lower right had a Class 2 Ramus Classification, and to the untrained inexperienced eye, it would seem easy to remove.

Given the sheer size of the tooth, elevating it too hard can lead to Fracture of the Mandibular Angle.

I did several sections of the crown to loosen the tooth and preserve the surrounding bone, so as to prevent Post Op Swelling and Possible Hematoma.

The Bilateral Impaction Surgery went very well, accomplished it under an hour, and I thank and praise the BIG BOSS UP THERE for guiding me.

AN INTERESTING CASE! Tagalugin natin para mas ramdam.

Itong pasyenteng ito ay galing sa isang clinic.

NABASA NYA AKO SA REDDIT AT SINUBUKAN MAGPA KONSULTA SA AKIN.

Madami ginawa ang Naunang Dentista sa kanya na umabot sa humigit ONE HUNDRED THOUSAND PESOS!

Ito ang mga ginawa:

1. Sinubukan magtanggal ng WISDOM TOOTH, pero inabot na ng gabi at hindi natanggal. Ayan ang "XRay" sa litrato, halos wala natanggal. Sabi pa ng pasyente ay ang daming pamanhid ang naiturok pero puro sakit pa din ang naramdaman nya. Ung sugat pa nga ay nanatiling bukas, hindi na nagsara.

Nagplano ako mabuti kung papano ko mapapabilis at komportable ang pagtanggal sa IMPACTED WISDOM TOOTH nya. Mabuti at natanggal natin ng under 45mins kasama na ang pagdokumento sa kaso.

2. Nag Root Canal ang Dentista ngunit di nakuhang matapos ng maayos, kaya inirekomenda na ipabunot na lang. Sa akin na din pinabunot ng pasyente ang nasabing ngipin.

3. Nagkabit ng Braces ang Dentista ngunit sa kasamaang palad, wala naging pagbabago. Ang malala, hindi makuha ang "UPPER MIDLINE" kaya nagbunot ng isang Ngipin, at nilagyan ng FIXED BRIDGE! Nasayang lang ang binunot at dagdag gastos pa ang pasyente.

Madami pa ako nakita na ginawa na hindi kaaya aya sa mata.

Hindi mura ang nagastos ng pasyente pero hindi nya natanggap ang tamang pag alaga at pag ayos sa Ngipin nya.

Isang malungkot na araw na naman para sa mga Kadentistahan.

Muli uulitin ko, maging mapanuri, piliing mabuti ang magiging Dentista mo!

ANOTHER DAY, ANOTHER FINISHED CASE!💪💪💪

The patient opted for a NON EXTRACTION APPROACH.

I was hesitant because I want to keep the NASOLABIAL ANGLE normal. With an EVIDENCE BASED APPROACH, i was able to accomplish that!

The Dental and Facial Midlines are coincident.

The Canines are now in Class 1 Position and so are the Molars!

No more Deep Bite. No more risks of having TMJ problems!

No more SEVERE crowding!
