🟣 Allergic Rhinitis vs Bacterial Sinusitis – How to Tell the Difference

Blocked nose, runny mucus, facial pressure β€” many people confuse allergies with sinus infection. But these are very different conditions and need different treatments. Let’s break it down clearly.

🟣 What Is Allergic Rhinitis?

Allergic rhinitis happens when your immune system overreacts to harmless substances like dust, pollen, pet hair, or mold.

β†’ It is NOT an infection
β†’ Triggered by allergens
β†’ Causes inflammation of nasal lining
β†’ Commonly called hay fever

🟣 Common Symptoms of Allergic Rhinitis

β†’ Clear, watery runny nose
β†’ Frequent sneezing
β†’ Itchy nose, eyes, or throat
β†’ Nasal congestion
β†’ Post-nasal drip (mucus flowing down throat)
β†’ Watery or red eyes
β†’ No fever
β†’ Facial pain is rare

Symptoms often come and go and may be seasonal or recurrent.

🟣 What Is Bacterial Sinusitis?

Bacterial sinusitis is an infection of the sinus cavities caused by bacteria. It usually starts after a cold or viral sinus infection that doesn’t heal properly.

β†’ Bacteria grow inside blocked sinuses
β†’ Pus collects in sinus spaces
β†’ Leads to pressure and inflammation

This condition often needs antibiotics.

🟣 Common Symptoms of Bacterial Sinusitis

β†’ Thick yellow or green nasal discharge
β†’ Facial pain or pressure (cheeks, forehead, around eyes)
β†’ Fever may be present
β†’ Blocked nose
β†’ Headache
β†’ Bad breath
β†’ Reduced smell
β†’ Fatigue
β†’ Symptoms lasting more than 10 days or worsening after initial improvement

🟣 Key Medical Differences

🟣 Cause
β†’ Allergic rhinitis: Allergen-triggered inflammation
β†’ Bacterial sinusitis: Bacterial infection

🟣 Nasal discharge
β†’ Allergic rhinitis: Clear and watery
β†’ Bacterial sinusitis: Thick yellow/green

🟣 Fever
β†’ Allergic rhinitis: Absent
β†’ Bacterial sinusitis: May be present

🟣 Facial pain
β†’ Allergic rhinitis: Rare
β†’ Bacterial sinusitis: Common

🟣 Duration
β†’ Allergic rhinitis: Seasonal or recurrent
β†’ Bacterial sinusitis: Persistent (>10 days)

🟣 Why This Difference Matters

Treating allergies with antibiotics does nothing.
Ignoring bacterial sinusitis can lead to complications.

Wrong treatment = delayed recovery.

🟣 How Doctors Diagnose

β†’ Clinical history and examination
β†’ Nasal examination
β†’ Sometimes CT scan (for complicated cases)
β†’ Allergy testing if symptoms are recurrent

Routine imaging is usually not needed for simple cases.

🟣 Basic Treatment Approach

🟣 For Allergic Rhinitis
β†’ Antihistamines
β†’ Nasal steroid sprays
β†’ Avoiding triggers
β†’ Saline nasal rinses

🟣 For Bacterial Sinusitis
β†’ Antibiotics (doctor prescribed)
β†’ Nasal decongestants (short term)
β†’ Pain relievers
β†’ Steam inhalation
β†’ Adequate hydration

⚠️ Antibiotics should only be used when bacterial infection is confirmed or strongly suspected.

🟣 When to Seek Medical Care Urgently

β†’ High fever
β†’ Severe facial swelling
β†’ Eye swelling
β†’ Vision changes
β†’ Severe headache
β†’ Symptoms lasting beyond 10 days
β†’ Worsening after initial improvement

These may indicate complications.

⭐ Key Takeaway

Clear watery nose + sneezing usually means allergy.
Thick yellow/green mucus + facial pain + fever lasting over 10 days suggests bacterial sinusitis.

Knowing the difference prevents unnecessary antibiotics and ensures proper care.

The National Nutrition Council (NNC) joins the nation in observing Goiter Awareness Week 2026, an annual campaign aimed at increasing public awareness on Iodine Deficiency Disorders (IDD) and promoting proper nutrition for optimal thyroid health.
Goiter is a preventable condition, primarily caused by inadequate iodine intake, yet it remains a public health concern affecting individuals across all ages. Left unaddressed, iodine deficiency can impair growth, development, and overall well-being.

This year, the NNC calls on individuals, families, communities, and local government units to take proactive action by ensuring adequate iodine intake through the use of iodized salt, consumption of a balanced diet, and regular health check-ups. Strengthening nutrition education and awareness is key to preventing goiter and other iodine deficiency–related disorders.

Together, let us work towards empowering every Filipino to protect their thyroid health and achieve overall wellness, as we support the shared vision of a healthier nation free from preventable nutrition-related conditions
