Upang makapag-avail ng mga benepisyo sa ilalim ng Philhealth YAKAP Program sa aming clinic, maari kayong makapag -register gamit ang egov app o di kaya pumunta sa nearest Philhealth Local Health Insurance office at sabihin ang aming Hernandez Health Hub Medical and Diagnostic Clinic ang iyong napiling Yakap provider.

Para sa mga nais magpa-rehistro gamit ang egovph app sundin lamang ang mga sumusunod.
1️⃣ I-download ang eGovPH App sa Google Play Store.
2️⃣ Mag-register at piliin ang aming klinika bilang inyong provider.
3️⃣ Mangyaring sundin nang mabuti ang mga instructions na makikita sa mga larawan sa ibaba.
4️⃣ Kapag napili na ang aming klinika, mag-screenshot ng proof na kami ang inyong selected provider at i-send ito sa aming page para sa verification o bumisita sa aming clinic at dalhin ang any government ID (not expired).
