“Building Bridges in Care through Coordination”

To strengthen partnerships and ensure seamless, patient-centered healthcare, the OURS Command Center Technical Working Group (TWG) — led by Chief of Medical and Professional Staff Dr. Alfonso Danac, with chairs and representatives from the Public Health Unit, Ugnayang HealthCare Patient Navigation, Outpatient Department, Emergency Department, IHOMS, and JASMC BUCAS Victoria Tarlac— met with the Tarlac Provincial Health Office under the leadership of Dr. Jeanette Lazatin on September 18, 2025.

The meeting also brought together representatives from district hospitals, Rural Health Units (RHUs), and Tarlac City, expanding the reach and impact of the discussion.

During this session, we opened communication lines with our partners and oriented HCPN members on the One Unified Referral System (OURS), eSKED (the OPD’s patient appointment system), and OK OPD, JBLMGH’s Telehealth service. We also began listing institutional accounts from Tarlac to facilitate their OURS access and created a directory of HCPN point persons to help sustain and strengthen the collaboration moving forward.

Through these efforts, we continue to C.O.N.N.E.C.T. with our partners in advancing better health services for our communities.
