Here’s how to request a replacement for your PhilID :

1. Visit any PhilSys Registration Center to personally report your peeled-off PhilID;
2. Present the peeled-off PhilID to the Screener;
3. Surrender your peeled-off PhilID and submit the accomplished Request Form; and
4. Wait for the text message from PSA for the release of your new PhilID.
