Mamaya na po ang launching ng Covid vaccination para sa mga kabataang 12 to 17 years old... para po sa PEDIATRIC A3 Category, narito po ang listahan ng mga comorbidities or ongoing na sakit na kabilang sa priority list. Para sa aming mga patients na may hika or ibang comorbidities, maari pong magpaschedule ng consultation sa aming secretary sa 0931 018 0182 :-)

Mga REQUIREMENTS- medical certificate or medical clearance mula sa inyong attending pediatricians, proof of guardianship na magpapatunay na kayo ang magulang ng mga babakuhan, confirmation email or barangay certification.

For those residents of San Pablo City: Here is the Form that anyone can download and print in a long typewriting paper. Fill it up and attach your 2x2inch size photo. You may directly submit this together with a xerox copy of your government ID to your respective Barangay Offices for encoding. They will then issue an ID that you will use when you go to centers or hospitals where the available COVID vaccines will be administered to you on certain schedules.
