Whats is Muscle Strain?

Muscle strains are tears to the fibers of muscles and tendons. A tear can occur at any part of a muscle or tendon. A tendon is a fibrous tissue that connects a muscle to a bone. The most common area to strain is at the junction where the muscle and tendon come together. The image above shows areas of the body where muscle strains can occur. The low back are is the most common strained muscles. Other common strains are your hamstring, quad, bicep, triceps, abdominal, pectoralis, calf and trap muscles.

Common muscles that get strained are the:
✅Hamstrings
✅Low back muscles
✅Neck muscles
✅Hip Flexors
✅Hip Adductors
✅Calf muscles (Gastrocnemius and Soleus)

How Do You Know if You have a Strained Muscle or Tendon?
A strained muscle or tendon usually causes some pain. The pain may be mild or it may be severe, depending on the severity of the strain. If the strain comes from trauma the pain may be instant. In mild cases, the pain of a strain may occur later or after an activity.

It is important to recognize the difference between pain and soreness in a muscle. After a tough workout or long run, it is typical to have soreness. Sometimes, the soreness will occur a couple of days after the activity. This is called Delayed Onset Muscle Soreness or "DOMS". DOMS can be very uncomfortable but it should resolve in a few days.

Sharp pain, deep ache, or loss of strength or control of a muscle are not typical symptoms of DOMS. These symptoms may indicate a strain.

Treatment may include:
✅Soft tissue manipulation
✅Graston Therapy
✅Ultrasound
✅Laser Therapy
✅Stretching
✅Strength training
✅Return to Sport Training

If the Strain is severe, or there is a loss of muscle control, an Orthopedic Surgeon may order an MRI to determine whether a surgical repair is needed. This is rare though. Most muscles strains respond very quickly to physical therapy. However, it is important to not return to quickly to a sport to prevent a reinjury.

Back pain is one of the most common reasons people seek medical help or miss work. Back pain is a leading cause of disability worldwide.

Fortunately, measures can help prevent or relieve most back pain episodes, especially for people younger than age 60. If prevention fails, simple home treatment and using the body correctly often will heal the back within a few weeks. Surgery is rarely needed to treat back pain.

Symptoms of Back Pain
Back pain can range from local pain in a specific spot to generalized pain spreading all over the back. Sometimes the pain radiates away from the back to other areas of your body, such as the buttocks, legs, or abdomen. The intensity of back pain varies for each person. Depending on the type, causes, and location of your back pain, you may experience:

✅ Increasing pain with lifting and bending.
✅ Worsening pain when resting, sitting, or standing.
✅ Back pain that comes and goes.
✅ Stiffness in the morning when awakening and lessened back pain with activity.
✅ Pain that radiates away from the back into the buttocks, leg, or hip.
✅ Numbness or weakness in your legs or feet.

Causes of Back Pain
Back pain can be caused by many different factors, which may all be present at the same time and interact to result in chronic low back pain. These could include mechanical or structural problems with the spine, inflammatory conditions, and other medical conditions. It is also possible that no specific cause can be identified for the start of back pain.

FACIAL EXERCISE FOR BELL'S PALSY

Bell’s palsy is when one side of the face becomes paralyzed. Movement of the eye, forehead, nose, and lips may be difficult if not impossible. However, along with medical treatment, physical therapy can help restore movement to the paralyzed face, just like physical therapy can help restore arm and leg movement for somebody who has suffered a stroke.

When there is no movement present at all, the following exercises are helpful in attempting to reinitiate some movement. It is important to focus on the effort or quality of the movement rather than the quantity of movement. The quality of effort is to really focus and concentrate HARD on trying to get the paralyzed side moving symmetrically with the normal side.

All exercises to be performed twice a day, 5-10 repetitions each time. And perform these in front of the mirror so you can visually monitor your efforts! The visual feedback is important!

WHAT IS BELL'S PALSY

Bell's palsy is a neurological disorder that causes paralysis or weakness on one side of the face. One of the nerves that controls muscles in your face becomes injured or stops working properly.

Symptoms include:

Sudden weakness or paralysis on one side of your face
A drooping eyebrow and mouth
Drooling from one side of your mouth
Difficulty closing an eyelid, which causes eye dryness
Most often these symptoms lead to significant facial distortions.

People living with Bell's palsy may also develop:
✅ Facial pain or abnormal sensations
✅ Excessive tearing in one eye
✅ Problems with taste
✅ Low tolerance for loud noises
✅ Pain around the jaw and behind the ear
✅ Problems eating or drinking

Symptoms appear suddenly over a 48- to 72-hour period and generally start to improve with or without treatment after a few weeks. They vary from person to person and can be mild to severe. Usually, you will recover some or all facial function within a few weeks to six months. Sometimes the facial weakness may last longer or be permanent.

Bell's palsy is the most common cause of facial paralysis, although its exact cause is unknown. It results from dysfunction of cranial nerve VII, which connects your brain to the muscles that control facial expression (the nerve also is involved with taste and ear sensation). In rare cases, Bell's palsy can affect both sides of your face.

Who is more likely to get Bell's palsy?
Bell's palsy can affect anyone of any gender and age but seems to be highest in those in people 15 to 45 years old. Risk factors for Bell's palsy include pregnancy, preeclampsia, obesity, hypertension, diabetes, and upper respiratory ailments.

Possible triggers of Bell's palsy may include:

An existing (dormant) viral infection
Impaired immunity from stress, sleep deprivation, physical trauma, minor illness, or autoimmune syndromes
Infection of a facial nerve and resulting inflammation
Damage to the myelin sheath (fatty covering that insulates nerve fibers)

Platelet-Rich Plasma (PRP) Injections
In recent years, doctors have learned that the body has the ability to heal itself. Platelet-rich plasma therapy is a form of regenerative medicine that can harness those abilities and amplify the natural growth factors your body uses to heal tissue.

What is plasma and what are platelets?
Plasma is the liquid portion of whole blood. It is composed largely of water and proteins, and it provides a medium for red blood cells, white blood cells and platelets to circulate through the body. Platelets, also called thrombocytes, are blood cells that cause blood clots and other necessary growth healing functions.

Platelet activation plays a key role in the body's natural healing process.

What is platelet-rich plasma (PRP) and what are PRP injections?
Platelet-rich plasma (PRP) therapy uses injections of a concentration of a patient’s own platelets to accelerate the healing of injured tendons, ligaments, muscles and joints. In this way, PRP injections use each individual patient's own healing system to improve musculoskeletal problems.

PRP injections are prepared by taking anywhere from one to a few tubes of your own blood and running it through a centrifuge to concentrate the platelets. These activated platelets are then injected directly into your injured or diseased body tissue. This releases growth factors that stimulate and increase the number of reparative cells your body produces.

Ultrasound imaging is sometimes used to guide the injection. The photographs below illustrate a PRP injection into a patient's torn tendon. The ultrasound guidance is shown at left and the injection is shown at right.

Platelet-rich plasma has been found to significantly enhance the healing process, and using a PRP injection for shoulder pain caused by rotator cuff tears, for Achilles tendon ruptures and for other soft-tissue injuries is becoming more common.

PRP has also been demonstrated to improve function and reduce pain in people who have tendonitis or chronic tendinosis conditions such as tennis elbow or golfer's elbow.

Some of the key advantages of PRP injections are that they can reduce the need for anti-inflammatories or stronger medications like opioids. In addition, the side effects of PRP injections are very limited because, since the injections are created from your own blood, your body will not reject or react negatively to them.

What are the Treatment Options for Stroke Patients?

A stroke requires emergency care. You will probably receive treatment in a specialized stroke unit of the hospital. A team of specialists are in charge of your care. Treatment depends on whether the stroke was ischemic or hemorrhagic, how much time has passed since symptoms started, and whether you have other medical conditions.

Treating an Ischemic Stroke
Treatment for an ischemic stroke or transient ischemic attack (TIA) may include medicines and medical procedures.

Medicines
The main treatment for an ischemic stroke is a medicine called tissue plasminogen activator (tPA). It breaks up the blood clots that block blood flow to your brain. A healthcare provider will inject tPA into a vein in your arm. This type of medicine must be given within 3 hours after stroke symptoms start. People who may still have large parts of the brain without major damage may benefit from tPA even after 4.5 hours, but research on this is still ongoing. Your provider may give you tPA up to 4.5 hours after your symptoms begin or if you woke up with stroke symptoms less than 4.5 hours beforehand. The sooner treatment begins, the better your chances of recovery. If you cannot have tPA, your provider may give you an anticoagulant or blood-thinning medicine, such as aspirin or clopidogrel. This helps stop blood clots from forming or getting larger. The main side effect of these medicines is bleeding.

Medical procedures
You may need a procedure to open up blocked arteries and restore blood flow to the brain. This can be done several ways.

A thrombectomy removes the clot from the blood vessel. A surgeon puts a long, flexible tube called a catheter into your upper thigh and threads it to the blocked artery in your neck or brain. They use angioplasty and stenting, or a device called a stent retriever, to open up the blocked artery.

Angioplasty and stenting procedures use a thin tube to insert a balloon or small mesh tube into the artery. Gently blowing up the balloon or expanding the mesh tube clears space for blood to flow more easily to the brain.

The stent retriever is a wire mesh inside the catheter that traps the clot. The stent retriever and the blood clot are then pulled out through the tube.

If carotid artery disease caused your stroke, your provider may suggest carotid endarterectomy, a surgery to remove plaque from the carotid artery in your neck.

Treating a Hemorrhagic Stroke
Hemorrhagic stroke can happen suddenly and grow worse quickly. Just like with ischemic stroke, getting treatment as quickly as possible is key to a full recovery. The type of treatment you receive depends on what part of your brain is bleeding and how much.

Medicines
Your healthcare provider may give you blood pressure medicine to lower the pressure and the strain on blood vessels in the brain. You will also stop taking any anticoagulant or blood-thinning medicines that may have led to bleeding. Depending on the type of medicine you take, your provider may give you vitamin K to help stop bleeding.

Medical procedures
There are several procedures that may be used.

Aneurysm clipping blocks off the aneurysm from the blood vessels in the brain. This surgery helps stop bleeding from an aneurysm. It can also help prevent the aneurysm from bursting again. During the procedure, a surgeon places a tiny clamp at the base of the aneurysm.

Blood transfusion replaces blood lost through surgery or injury. A blood transfusion is a common, safe medical procedure in which healthy donor blood is given to you through an intravenous (IV) line inserted in one of your blood vessels.

Coil embolization blocks blood flow to or seals an aneurysm. The surgeon will insert a flexible tube called a catheter into an artery in the upper thigh. They will thread the tube to the aneurysm in your brain. The surgeon will push a tiny coil through the tube and into the aneurysm. The coil will cause a blood clot to form, which will block blood flow through the aneurysm and prevent it from bursting again.

Draining excess fluid that has built up in the brain after a stroke and pushed the brain against the skull, causing damage, is another treatment. Draining the fluid can relieve that pressure.
Surgery may be performed to temporarily remove part of the skull if you have a lot of swelling. This allows room for the brain to swell without putting pressure on it.

Surgery or radiation may be used to remove or shrink an arteriovenous malformation (AVM). An AVM is a tangle of arteries and veins that can break open in the brain.
Surgery to remove pooled blood may also be used. Typically, your provider will use surgery only if you show signs of getting worse.

Other care you may receive in the hospital
In addition to treating the blockage or bleeding causing the stroke, your healthcare provider may suggest additional treatments or tests.

Breathing support may be necessary. If your stroke makes it hard to breathe or your oxygen levels are low, you may receive ventilator support.

Compression therapy using an air-filled sleeve on your leg can lower the risk of venous thromboembolism.

A feeding tube may be used if it is hard for you to swallow on your own. The tube provides you with nutrients.
Fluids may help restore proper blood pressure or blood volume if those levels are low.

Medicine may be used to lower a fever. Your provider will watch your body temperature and may give you acetaminophen or another medicine to lower fever and prevent additional brain damage.

Rehabilitation plans may be recommended. Before you leave the hospital, your provider will test how well you can speak, swallow, and walk. You and your provider can work together to set up a rehabilitation plan.

Skin care will help prevent skin irritation or sores from forming. Your provider will help make sure that you have enough cushioning, your skin stays dry, and you change positions often if you cannot move well on your own.

Stroke is a life-threatening condition that can cause severe disability. palliative care or hospice care may help some people have a better quality of life with fewer symptoms.
