In a newly published 2026 ACVIM-endorsed consensus statement and systematic review, updated, evidence-based recommendations are provided for the classification, diagnosis, and management of chronic inflammatory enteropathy in dogs.
A central clinical recommendation is the prioritization of dietary therapy. Dietary treatment trials are recommended as the preferred first-choice diagnostic approach in dogs suspected of having CIE before invasive diagnostic procedures are performed, provided patients are clinically stable and not hypo- or anorexic.
Because CIE is subclassified based on response to treatment, dietary modification serves both diagnostic and therapeutic roles. Published data indicate that approximately 38% to 89% of dogs with CIE are food-responsive, and many maintain clinical remission for at least three months on dietary therapy alone.
Beyond dietary management, the consensus statement also addresses stepwise diagnostic evaluation, the role of histopathology and biomarkers, considerations surrounding antimicrobial stewardship, and the use of immunosuppressive therapy in refractory disease. Collectively, these recommendations emphasize a structured, evidence-based approach to the diagnosis and long-term management of chronic gastrointestinal disease in dogs.
