Patient Requirements:
To ensure a smooth and efficient treatment process, please bring the following documents and information:
- Hemodialysis Endorsement
- Latest laboratories: Hepatitis Profile (HBsAg, anti-HBs, anti-HCV), CBC, Creatinine, BUN, BUA, Na, K, iCa, Phos, Chest Xray
- Copy of 3 recent dialysis treatment record from previous facility
- Philhealth Membership Data Record
- Certificate of Philhealth use from previous facility
- Immunization and vaccination record (Hep B, Flu, Pneumonia, COVID)
