This 2 year-old male came in due to a visible mass noted at the bridge of the nose. Further work up revealed a nasoethmoidal encephalocoele (NEM); he underwent repair of NEM where we removed the herniated brain and covered the defect at the skull base. After 2 weeks , this little cute boy followed up with unremarkable recovery.

Nasoethmoidal encephalocoele is a rare congenital defect where a portion of brain herniate through a defect in the skull. It is usually seen as a visible mass at the bridge of the nose. Surgical treatment includes removal of the herniated brain and covering the skull defect.

Interesting finding.

We performed the first endoscopic transphenoidal pituitary surgery in a private hospital (set up) in Tagbilaran City, Bohol for a patient who complaint of headache and blurring of vision.

Biopsy confirmed to be a pituitary tuberculoma, an extremely rare condition, even in an area where tuberculosis is endemic.

Fortunately, it responds well to our anti-tuberculosis medications.

Kababayan.

A 34/F came in due to 1 year history of loss of smell and right eye blindess. MRI taken which revealed a giant olfactory groove meningioma.

Patient underwent bifrontal craniotomy, gross total resection of tumor. Patient is currently underway to recovery.

Back to back aneurysm surgeries

First case:
A 49/M came in due to deteriorated sensorium. CT scan revealed an left intraparenchymal bleed with subarachnoid hemorrhage. Patient underwent emergency left decompressive hemicraniectomy, exploration and clipping of ruptured left MCA M1 aneurysm

Second case:
A 66/M came in due to unresponsiveness. CT angiogram revealed a saccular aneurysm at the anterior communicating artery. Patient underwent left pterional craniotomy, clip-reconstruction of ruptured aneurysm.

Case point:

A 49/M came in due to difficulty ambulating (dragging his left foot while walking), and numbness on lower extremities. MRI shows intradural, extramedullary lesion at the thoracic 2 and 3.

Patient underwent laminectomy, excision of tumor. Patient dramatically improved after 48hrs and was able to normally walk at post op day 3.

Case point:

A 58/M, came in due to headache described as “worst headache of my life”.

CT scan revealed diffuse aneurysmal subarachnoid hemorrhage. CT angiogram revealed ruptured aneurysm at the anterior communicating artery and unruptured aneurysm at the left MCA Bifurcation.

Emergency craniotomy, clipping of aneurysms was done.

Although aneurysm is commonly found in females, increasing cases of cerebral aneurysm in males are brought about by factors such as uncontrolled hypertension and smoking.

Ayon sa Private Hospitals Association of the Philippines Inc., may mahigit P530 million pa na hindi nababayarang claims para sa mga serbisyong medikal na naibigay na.

Dagdag nila, isa sa 43 pribadong ospital sa Batangas ang naghihintay ng bayad na P94 million.

Case point:

A case of a 46/F, hypertensive and poorly compliant to medications, sought consult due to severe headache, nausea and vomiting. CT scan and CT angiogram of the brain are consistent with a finding of a ruptured aneurysm. Patient underwent craniotomy, clipping of ruptured aneurysm and was discharged improved after 5 days.

Women have higher incidence of both ruptured and unruptured aneurysm compared to men. Most commonly, women who suffered from ruptured aneurysm are hypertensive. Risk of developing aneurysm is even higher in post-menopausal women

A 28-year old male came in due to immobility for 3 months due to severe back pain. MRI noted spinal lesion that affected his thoracic spine.

He underwent spine instrumentation to support his spine. He was discharged with improvement of pain and able to sit after 5 days.

2 months after surgery, he is now able to walk, with minimal assistance of walker.

What a blessed day to sing praises to our Lord. God is good, all the time.
