TERMS AND CONDITION:
▪️ Patient must accomplish the HMO Online Google Form to register
▪️ CGTDMC HMO Online Approval is available from Monday-Sunday (8AM-5PM)
▪️ Status of request approval will be sent via SMS/Viber/Email within 24 hours upon request.
▪️ Approved LOA may claim at CGTDMC HMO Department at the Ground Floor Level.
