💥Cervical Radiculopathy:

It is a condition where the nerves in the neck get irritated or compressed, causing pain, weakness, or numbness that can extend into the arms or hands.
➡️Causes:
It's often caused by pressure on the nerves due to herniated discs, bone spurs, or degeneration of the cervical spine.
➡️Symptoms:
Common symptoms include neck pain, shooting pain down the arm, weakness in the muscles, numbness or tingling in the arms or hands, and sometimes difficulty with coordination.
➡️Special Tests:
Doctors may perform physical exams, MRI scans, or spurling test to diagnose cervical radiculopathy and determine its severity.

➡️Treatment:
⏮️Conservative Treatment: Often starts with rest, physical therapy, and medications to reduce pain and inflammation.
⏮️Physical Therapy: Involves exercises and stretches to improve posture, strengthen muscles, and relieve pressure on the nerves. This may include:
✔️Neck stretches and range of motion exercises.
Strengthening exercises for the neck, shoulders, and upper back.
✔️Postural exercises to improve alignment and reduce strain on the neck.
✔️Manual therapy techniques like massage or manipulation to relieve muscle tension and improve mobility.
✔️Modalities such as heat or ice therapy, ultrasound, or electrical stimulation to reduce pain and promote healing.
➡️Medications: Nonsteroidal anti-inflammatory drugs (NSAIDs), muscle relaxants, or pain relievers may be prescribed to manage pain and inflammation.
➡️Injections: Steroid injections into the affected area can provide temporary relief from pain and inflammation.
➡️Surgery: In severe cases where conservative treatments fail, surgery may be recommended to relieve pressure on the nerves.

✔️A rotator cuff tear is an injury to one or more of the tendons or muscles that help stabilize and move the shoulder joint. The rotator cuff is made up of four muscles - supraspinatus, infraspinatus, teres minor, and subscapularis.
➡️Causes
🔹Repetitive overhead motions and activities like pitching, swimming, tennis, lifting heavy objects
🔹Aging and degeneration of tendons
🔹Trauma or fall on shoulder
🔹Bone spurs or calcification in shoulder area
➡️Symptoms
🔹Pain in shoulder, especially with overhead motions
🔹Weakness and loss of motion in shoulder
🔹Difficulty sleeping on affected side
🔹Crepitus or crunching sound with shoulder movement
➡️Diagnosis
It involves medical history, physical exam (tests for weakness/pain), and imaging like MRI
✔️Special Tests
🔹Neer test -
🔹Hawkins-Kennedy test
🔹Empty can test:
🔹Drop arm test:
➡️Treatment
✔️Physical therapy
🔶Early Stage Rehab:
🔹Rest, ice, compression to reduce pain and inflammation
🔹Gentle range of motion exercises like pendulum swings to prevent stiffness
🔹Avoid aggravating motions like overhead arm lifting or reaching behind
🔶Intermediate Stage:
🔹Progressive strength training for rotator cuff and scapular muscles
🔹Targeted stretches and exercises to improve range of motion
🔹Manual therapy like mobilizations to improve shoulder mechanics
🔹Modalities like heat/ice, electrical stimulation for pain relief
🔶Advanced Stage:
🔹Advanced isotonic and isokinetic exercises with resistance bands
🔹Functional training for overhead activities and sport-specific motions
🔹Proprioception and dynamic stabilization exercises
🔹Address any muscle imbalances or movement compensations
Prepare patient for return to activities/sports with adequate strength and control
The focus is on protecting the shoulder, allowing any torn tissues to heal, regaining motion and strength, and ultimately return to full function.
✔️NSAIDS to reduce pain and inflammation
✔️Corticosteroid injection to decrease inflammation
✔️Surgery like debridement, repair or reattachment of torn tendon
If no repair possible, shoulder joint replacement may be considered

➡️Frozen Shoulder(کندھے کا جام ہونا)
Frozen shoulder, also called adhesive capsulitis, is a condition where the shoulder becomes stiff and painful, with restricted range of motion.

🔶 Stages:

🔹Freezing Stage: gradual onset of pain and stiffness in the shoulder, may last from 6 weeks to 9 months.
🔹Frozen Stage : shoulder becomes very stiff and painful, range of motion is severely limited, can last from 4 to 12 months.
🔹Thawing Stage:pain starts to decrease, shoulder stiffness gradually improves, can last from 12 to 42 months.

🔶Causes:

🔹 Injury or trauma to shoulder.
🔹 Diabetes.
🔹Hypothyroidism.
🔹Immobilization of shoulder.
🔹 Rotator cuff injuries.

🔶Symptoms:

🔹Pain in shoulder and arm
🔹Limited range of motion,
🔹Difficulty sleeping on affected side
🔹Pain with movement.

➡️Treatment:

🔶Physiotherapy:

🔹Passive joint mobilization: The physiotherapist will gently move the shoulder joint to improve its mobility. This helps stretch the tightened tissue.
🔹Active assisted exercises : The physiotherapist assists the patient in moving their shoulder through its range of motion. This helps maintain mobility.
🔹Active exercises :The patient performs exercises to improve strength, mobility, and function of the shoulder. May include pendulum exercises, wall walks, pulley exercises.
🔹Stretching - Gentle stretches that target the shoulder joint capsule and muscles. Hold stretches for 30 seconds, repeat 5 times.
🔹Manual therapy - Massage, trigger point release, and other hands-on techniques to loosen muscles.
🔹Heat or ice therapy - Heat before exercise helps relax muscles. Ice after exercise helps reduce pain and swelling.
🔹TENS
🔹Ultrasound
🔹Short Wave Diathermy
🔹Patient education - Teaching proper posture, joint protection, and home exercise program.
🔹Return to normal activities - Gradually increase use of shoulder for normal activities as able

🔸Medications - NSAIDs, cortisone injections, nerve blocks.
🔸Surgery - manipulation under anesthesia to break up adhesions (in severe frozen stage).
Home exercises and maintaining range of motion are important during recovery.
