This Young boy had a history of Shot gun injury presented to me in ER , Profusely Bleeding and in the state of hypovolemic shock. After a Primary survey and initially stabilizing him he was taken to OR.
All necrotic, burned and avulsed.
His half of the face was gone after debridement.
These type of injuries are very difficult yet critical to manage because Surgeon has yo keep many factors in mind. Minimum surgical trauma, prevention of anticipated infection, tension free closure so as to minimize chances of post op dehiscence ( to ensure that in this case i had to make many advancement rotation flaps for soft tissue closure), and to ensure best possible outcomes in terms of aesthetics post op.
Unfortunately intra oral condition was not that good total loss of labial and buccal vestibules. Fortunately no bony injury.
Intra orally i have given him WHV pack and will be addressed later on with scar revisions.
This is the picture of his 3rd post operative day, up till now all is good with no signs of infection.
Let's pray for his smooth and uneventful complete recovery.
