1. MBBS degree attested
2. Pmdc attested
3. Matric pacca certificate attested
4. Inter pass certificate from board attested
5. CNIC attested copy
6. Challan (if paid manually)
7. Domicile attested
8. PRC c form attested
9. D form attested
10. Copy of online application form along with applicant signature
11. Three attested photographs (attested on back side)
12. Graduation pass certificate attested
13. All MBBS mark sheets or consolidated/transcript attested
14. NOC (if already in govt job)
15. Disability certificate for differently abled persons quota mandatory
All attested documents by Grade 17 officer ..

All candidates are advised to submit their required documents this week. Candidates who have any missing documents should submit the available ones within the given time frame.

The SPSC will issue a deficiency list after the initial document review. Candidates listed in the deficiency list will be granted 7 days from the date of issuance to submit the missing documents.
