Have you ever heard about Sever's Disease?
Sever's disease, also known as calcaneal apophysitis, is a common heel condition that affects children and adolescents during periods of rapid growth.

Despite its name, Sever's disease is not actually a disease, but rather an overuse injury that occurs at the growth plate in the heel bone (calcaneus).

It is most commonly seen in active children between the ages of 8 and 14 years, who are involved in sports or physical activities that involve repetitive impact on the heel, such as running, jumping, and playing sports like soccer or basketball.

The exact cause of Sever's disease is not fully understood, but it is believed to result from repetitive stress and tension on the growth plate in the heel.

During growth spurts, the bones in the body grow faster than the surrounding muscles and tendons, which can create tension at the growth plate. This tension can cause inflammation, pain, and swelling in the heel.

Symptoms of Sever's disease typically include pain and tenderness in the heel, especially during physical activity or when pressure is applied to the area.

Treatment for Sever's disease is usually conservative and focuses on relieving pain and reducing inflammation.

Some common treatment options include rest, ice, supportive footwear, stretching and strengthening, orthotic devices, pain medications, physiotherapy.

However, it may take several months or even up to a year for symptoms to fully resolve.

It is important to seek medical attention if your child is experiencing persistent or worsening heel pain to ensure proper diagnosis and appropriate treatment.

♻️ISCHEMIC STROKE

A stroke is an accidental trauma to the brain often following absence of blood flow to the brain.This obstruction in blood flow to the brain can be due to a blockage of an artery which supplies part of the brain with blood. A cerebral artery can also rupture as a result of high blood pressure.This could also stops blood flow to part of the brain, which can also cause stroke.

Whenever there’s a blockage in the lumen of an artery which supplies the brain with blood, the clot or plaque stops blood flow to part of the broke. An ischemic stroke is not different from this occurrence. It is a type od stroke which occurs when there is a blockage in a cerebral artery or an artery elsewhere in the body and travelled along to the brain, stopping blood flow. It’s the most common type of stroke, and around 85% of strokes in the UK are ischaemic strokes.The other 15% of strokes are due to bleeding in or around the brain, known as haemorrhagic stroke.

🔰CAUSES/RISK FACTORS

Blocked brain arteries cause ischemic strokes and TIAs. Any condition that leads to increased fatty plaques in the blood vessels, or blood clots in the bloodstream, can increase the risk of ischemic stroke and TIAs. Conditions include:

•Atherosclerosis (hardening of the arteries): Fatty buildup (plaque) on artery walls narrows the arteries (stenosis) and increases the risk of plaque breaking off. It may also cause platelets to form where the plaque damages arteries, increasing the risk of a blood clot or blockage.

•Carotid artery disease: Plaques forming in the carotid arteries (major arteries in the neck that supply blood to the brain) are a main cause of ischemic stroke and TIAs.

•Atrial fibrillation (rapid, irregular heartbeat): This and other heart conditions can create blood clots that may lead to an ischemic stroke or TIA. Atrial fibrillations are a common cause of embolic (blood clot) strokes.

🔰RISK FACTORS OF ISCHEMIC STROKE

🌀Cholesterol levels: High levels of “bad” low-density lipoprotein (LDL) cholesterol or low levels of “good” high-density lipoprotein (HDL) cholesterol can contribute to plaque buildup in the arteries.

🌀A sedentary lifestyle: A lack of physical activity can lead to high blood pressure, high cholesterol, and being overweight. These increase the risk of arterial plaque buildup.

🌀Being overweight or obese.

🌀An unhealthful diet: Consuming too many saturated or trans fats and foods high in cholesterol, sodium, and sugar can lead to diabetes, plaque buildup, high blood pressure, and high cholesterol.

🌀Being over 55 years of age or older: The risk of stroke increases in people aged over 55 years and increases with each additional decade of life.

🌀People who have had a previous transient ischemic attack (TIA) are also at risk of ischemic stroke. A TIA, or “mini-stroke,” is a temporary blockage of blood to the brain.

♻️SYMPTOMS
The symptoms of an ischemic stroke can involve one or more of the following:

1. One-sided weakness or paralysis.
2. Aphasia (difficulty with or loss of speaking ability).

3. Slurred or garbled speaking (dysarthria).
4. Loss of muscle control on one side of your face or facial droop.
5. Sudden loss, either partial or total of one or more senses (vision, hearing, smell, taste and touch).

6. Blurred or double vision (diplopia).
7. Loss of coordination or clumsiness (ataxia).
8. Dizziness or vertigo.
9. Nausea and vomiting.
10. Neck stiffness.
11. Emotional instability and personality changes.
12. Confusion or agitation.
13. Memory loss (amnesia).
14. Headaches (usually sudden and severe).
15. Passing out or fainting.
16. Coma

SERRATUS ANTERIOR WEAKNESS AND WINGED SCAPULAR.

Clinically, winging of the scapular can be attributed to weakness in the serratus anterior muscle. This is a muscle that originates from the first rib to the 9th rib. It is saw tooth like and hence, the name serratus.

The serratus anterior muscle, in combination with other muscles acts to protract your scapular. The rhomboid muscle, together with other muscles also helps retracts your scapular. Weakness in the serratus anterior muscle therefore makes the antagonistic muscles overactive and cause more scapular retraction. This is called winging of scapular or winged scapular.

The winged scapular is mainly the result of weakness in the scapular protractors, which serratus anterior muscle is the primary muscle.

This muscle is innervated by the long thoracic nerve and injury to this nerve result in weakness in the corresponding muscle, the serratus anterior muscle. There are several ways this long Thoracic nerve is injured. Long thoracic nerve is more prone to injury due to its excessive length.

Usually, injury to this nerve occurs due to trauma, direct blow to the rib area, over stretching or strenuous repetitive movements of the arms, and sustained bearing of excessive weight over the shoulder. It is when there’s injury or problem with the long thoracic nerve where the serratus anterior muscle will become weak. It is very clear and unquestionable to make a straight assertion that, injury to the C5-C7 root is the main cause of winged scapular. This is the root where the nerve in question can be traced to.

It is important to therefore make thorough examination to determine the actual problem first before making a judgment.

عام طور پر یہ سبز پتوں والی سبزیوں میں پایا جاتا ہے۔
حمل کے دوران یہ ایک صحت مند دماغ اور بچے کے ریڑھ کی ہڈی کی تشکیل کے لیے بہت ضروری ہے۔
دو وجوہات کی وجہ سے اس کی کمی ہو سکتی ہے۔
1.. ماں کی خوراک ہری پتوں والی سبزیوں میں مناسب نہیں ہے جو فولک ایسڈ کا بھرپور ذریعہ ہیں
2... ماں دوائیں لے رہی ہے جیسے مرگی کے علاج کی دوائیں جو اس کی کمی کا سبب بن سکتی ہیں۔
اگر اس وٹامن کی کمی ہو تو بچے کو کیا نقصان ہوگا؟
بچے کی کھوپڑی کھلی ہو سکتی ہے۔
بچے کا سر بڑا ہو سکتا ہے جو پانی سے بھرا ہوا ہے۔
بچے کو ریڑھ کی ہڈی میں زخم ہو سکتا ہے۔
اس لیے ماں کے لیے اس وٹامن کا اچھی مقدارمیں ہونا بہت ضروری ہے۔ہم تجویز کرتے ہیں کہ حمل کی تیاری کے دوران ہر ماں کو یہ فولک ایسڈ گولی حمل کے پہلے 3 ماہ میں لینا چاہیے۔اہم بات یہ ہے کہ حمل ٹیسٹ مثبت آنے کے بعد اگر آپ اسے لیتے ہیں تو اس کا کوئی فائدہ نہیں کیونکہ پہلے ہی بچے کا ایک ماہ گزر چکا ہوتا ہے کیونکہ حمل ٹیسٹ مثبت آنے میں عام طور پر ایک ماہ کا وقت لگتا ہے۔اس لیے اس وٹامن کو اپنی بچے کی تیاری کی منصوبہ بندی کے مطابق لینا شروع کر دیں اور ٹیسٹ کے مثبت ہونے کا انتظار نہ کریں۔یہ بہت اہم معلومات ہیں جو کسی کی زندگی کو بچا سکتی ہے اور خاندانوں میں عمر بھر کے معذور بچے پیدا کرنے سے روک سکتی .

ڈپریشن کیا ہے ؟

کسی چیز کو بار بار سوچنا
کسی بات کے ہونے کا خوف ہونا,
خود کا دوسروں سے مواذنہ کرکے احساسِ کمتری ہوجانا, کسی چیز کی خواہش کرنا اور اُس کے نا ملنے پر پریشان ہوجانا, نا اُمید ہوجانا اور مایوس ہوجانا....

ڈپریشن کب ہوتا ہے ؟

جب ہم کسی بات کو لے کر پریشان ہوتے ہیں اور بناء کسی کو بتائے مسلسل اُسے ہی سوچتے رہتے ہیں....ہمیں اچھے کی اُمید نہیں ہوتی ہمیں لگتا ہے کہ بس اب حالات ایسے ہی رہینگے تب ہم ڈپریشن میں چلے جاتے ہیں....

ڈپریشن کا علاج کیا ہے ؟

ڈپریشن کا %80 علاج آپ نے خود کرنا ہے....
جو بات آپ کو پریشان کر رہی ہے اُسے کسی بھروسے مند شخص سے discus کریں جیسے آپکا کوئی اپنا ماں باپ, بہن بھائی.... اگر اُن سے نہیں کر سکتے تو اللہ سے باتیں کرنا شروع کر دیں روز رات کو اُسے الف سے ے تک کی کہانی سُنا کر سونے کو اپنا معمول بنا لیں.....
آپ ہمیشہ سے تو اس پریشانی میں نہیں تھے وقت بدلتا ہے چاہے خوشی ہو یا تکلیف تو یہ وقت بھی گُزر جائے گا.... ہمیشہ اپنے سے کمتر لوگوں کو دیکھیں اور اُن کی محرومیوں کو جان کر اپنے ارد گرد بے شمار نعمتوں کا شکر ادا کریں, دوسروں کو سُننا شروع کر دیں اور پھر آپ اس بات کو سمجھ جائیں گے کہ اس دنیا میں صرف آپ اکیلے ہی پریشان نہیں ہیں....

🔈CERVICOGENIC HEADACHE (CGH) Syndrome

• Chronic hemicranial pain that is referred to the head from either bony structures or myofascial soft tissues of the upper CSP, - OA, C1-3 segments.

• The C1-C3 nerves relay pain signals to the nociceptive nucleus of the head and neck, the trigeminocervical nucleus. This connection
is thought to be the cause for referred pain to the occiput and/or eyes.
• The trigeminocervical nucleus is a region of the upper cervical spinal cord where sensory nerve fibres in the descending tract of the trigeminal nerve (trigeminal nucleus caudalis) are believed to
interact with sensory fibres from the upper cervical roots. This functional convergence of upper cervical and trigeminal sensory pathways allows the bidirectional referral of painful sensations
between the neck and trigeminal sensory receptive fields of the face and head.
• A functional convergence of sensorimotor fibres in the spinal accessory nerve (CN XI) and upper cervical nerve roots ultimately converge with the descending tract of the trigeminal nerve and
might also be responsible for the referral of cervical pain to the head.

Important to differentiate between: Migraine without aura, Tension-type, and Cervicogenic
headache.

• In Cervicogenic Headache, patient may present with combination of:
- movement dysfunction in whole of Csp
- muscle function impairment
- upper cervical individual joint dysfunction
• Pain and tenderness in sensitive structures in the upper neck, fascia in the head, TMJ,
mandible.
• History is important, look for activities involving some repetitive movement
static posture, maybe previous injury.
• Usually Ipsilateral pain but it can be bi-lateral!
• Patient can report unsteadiness and light-headedness, but fainting is a red flag!
