Guidelines on ‘Preventing Medication Errors – Ensuring Medication Safety’
The medication management and use of medication is a complex process, which involves multiple professional tiers besides the pharmaceutical field, the systems and the community itself. The Guidelines on ‘Preventing Medication Errors – Ensuring Medication Safety’, developed by the Punjab Healthcare Commission, provide practical recommendations and best practices for healthcare providers in preventing and mitigating medication errors in hospitals and healthcare settings. Reporting and learning systems have important roles in eliminating and minimising preventable medication errors.
The primary objective of the Guidelines is to prescribe a good practice guide on recording, coding, reporting, and assessment of medication errors to obtain information and maintain a database on the occurrence of all medication errors that may compromise patient safety. The purpose of the Guidelines is to explain the issues related to medication errors, including the causes of errors and prescribe strategies for eliminating the errors through implementing regulatory provisions and good practices.
