•Lateral injury to major bile duct (Strasberg type D injuries) can also be approached in the early postoperative period. These injuries are also amenable to endoscopic sphincterotomy and stenting. Of note, if a partial transection of the common bile duct is recognized at the time of initial surgery, it can be repaired primarily over a T tube. Fine, monofilament, absorbable sutures should be used for the repair. The T tube should be brought out of the common bile duct at a distant site away from the repair site (Fig).
👉•Primary end-to-end repair of a bile duct injury over a T tube. In general, this technique is used for partial transections of the bile duct, when there has been no associated loss of ductal length. Note that the T tube does not exit at the site of injury.
•If the injury is secondary to the use of cautery or results in complete transection of the duct, results of primary repair are poor. One study reported a restricture rate of nearly 100% for end-to-end repairs of the common bile duct. These patients are best managed with a biliary-enteric anastomosis as laterdescribed.

Can you discharge a patient with seat belt sign?

Traditional trauma teaching is to admit trauma patients with abdominal wall ecchymosis caused by seat belts (seat belt sign) for fear of missing a hollow viscus injury leading to peritonitis and sepsis.
Over the past few years there have been studies pointing toward the safety of discharging blunt abdominal trauma patients with a negative CT even if they do have a seat belt sign.
In this most recent study, a negative CT was defined as
1. No free fluid (free fluid was the leading indicator of occult hollow viscus injury)
2. No solid organ injury
3. No bowel wall irregular contours, thickening, hematoma or air
4. No abdominal wall soft tissue contusion
5. No mesenteric stranding or hematoma
6. No bowel dilatation
If the patient’s CT did not include any of these findings, there was a 0.01% chance of finding a delayed hollow viscus injury. The authors conclude it is safe to discharge patients meeting these criteria.
If we include no rebound or guarding on physical exam along with a negative CT scan, it appears to be safe to discharge trauma patient’s with seat belt sign.

آٹھویں جماعت تک کا ہر بچہ اپنے شوق کے مطابق کم از کم کسی ایک ہنر میں مہارت پیدا کر چکا ہو جس کا مڈل کے امتحان میں پریکٹیکل امتحان لیا جائے اور کامیاب امیدوار کو کام کرنے کا سرٹیفکیٹ/ڈپلومہ جاری کر دیا جائے ۔ جو بچے اس اسٹیج پر پریکٹکل لائف میں چلے جائیں ان کے لیے آگے پڑھائی جاری رکھنے کے لیے نائٹ اسکولز کا اجرا کیا جائے نیز ان نائٹ سکولز میں داخلہ، پریکٹیکل ٹریننگ ڈپلومہ کی بنیاد پر کیا جائے ۔ کتب اور ٹیوشن مفت ہو یا بس ٹوکن فیس منی کے نام پر چند روپے جو ان ہونہار طالبعلموں کی خودداری کی ضمانت ہو۔
میٹرک تک کی تعلیم حاصل کرنے والے طلبہ اسی ہنر میں سکول سے ہی گریجوایٹ ڈگری حاصل کریں ۔ کالج کی روایتی تعلیم جاری رکھنے کے امیدوار ہنرمند کے لیے اس کی متعلقہ فیلڈ مثلا الیکٹریشن پلمبنگ یا لیب ٹیکنیشن، ٹیلرنگ، بیوٹیشن وغیرہ کی اسپشلائزڈ ٹریننگ کا انتظام کیا جائے ۔
کالج سے بی اے کرنے والا ٹیکنیشن یا الیکٹریشن اپنی متعلقہ فیلڈ میں پوسٹ گریجوایٹ اسپیشلسٹ کے لیول پر تسلیم کیا جائے اور ان کی ڈگری میں مینشنڈ ہو کہ وہ اپنی فیلڈ میں دنیا کے کسی بھی مقام پر مہارت کے ساتھ کام کرنے کے اہل ہیں ۔
طب اور انجینئیرنگ ہی کی مانند دیگر ممالک میں ہمارے ان ہنرمندوں کے لیے روزگار کے دروازے کھولنے کی کوشش، بہترین خارجہ پالیسی کے ساتھ ممکن بنائی جائے ۔ ڈاکٹرز کی مانند یہ ہنر مند بیرون ملک ،ایک امتحان کے زریعے اپنی قابلیت ثابت کریں اور حسب قابلیت جاب، عہدہ اور تنخواہ و مراعات پائیں ۔ ہمارا وطن بھی ہنر مند افراد کی مد میں خودکفیل ہو اور دنیا کو بھی ہنر ایکسپورٹ کرے جس سے پاکستان کو زرمبادلہ حاصل ہو۔

جو تجاویز میں نے لکھی ہیں میں اپنے طور پر اپنے مریضوں اور ملنے جلنے والوں میں اس کی ترویج کرتی رہتی تھی بہت سے بچے جو عمومی پڑھائی سے جی چراتے تھے ان میں سے چند ایک اے سی ٹیکنیشنز ، کار مکینک، ٹیلر ماسٹر اور بیوٹیشن بن کے اپنا روزگار کما رہے ہیں ۔ یہ خیال ہر ذی شعور کے ذہن میں اتا ہے کہ سادہ ایف اے، بی اے، ایم اے کا حاصل کیا ہے جب تک کوئی ہنر ہاتھ میں نہ ہو ۔

گراس روٹ لیول پر تعلیمی نظام تبدیل کرنے کی ضرورت ہے کہ محض رٹو طوطے پیدا کر ے کی بجائے باہنر افراد تیار کیے جائیں کیونکہ
تونگری بہ ہنر است ۔۔۔

17 Types Of Sadaqah Every Muslim Man and Woman Should Know.

Sadaqah is not only linked with money. We often think that taking out money from our earnings are Sadaqah, but there are many ways we can do it!
These are the 17 types we can take out Sadaqah;

1. Dua
If you are just praying for the person who you care about or any person who has asked for you dua.

2. Knowledge.
Spreading knowledge among the people who can’t afford it.

3. Advice.
Giving wise advice to your younger siblings or people who are younger than you.

4. To smile at your Muslim brother.
Talking, meeting and greeting the people you meet.

5. Help.
Help the poor and help the people who can not help themselves. Solving people problems can also be the thing you can do.

6. Time.
Taking time out for the parents and your wife/husband is also a sadaqah.

7. Tarbiyyah.
Nurture (Tarbiyyah), growing up your kids well mannered is also a sadaqah.

8. Patience over difficult times.
Be very patient, do not yell about your problems to anyone. Allah will solve them for you.

9. To advise for good.
Advice your misguided friends to be on the Path of Allah.

10. To stop from evil.
Do not plan the bad thing, do not hurt or kill any living thing.

11. To talk softly.
Do not be harsh and Rude, Allah does not like Arrogant people.

12. To forgive.
Forgive the people who ask for your forgiveness.

13. To give respect.
Giving respect to elders and youngsters as well as giving credit to the people who deserve it.

14. To be a part of someone’s happiness.
Being happy in someone else’s happiness, DO NOT BE JEALOUS!

15. To visit the sick.
Visiting the sick is also the sunnah of our beloved Prophet (SAWW)

16. To remove harmful things from the path.
Such as removing the stone that comes to your way.

17. To guide someone on the right way.
Guiding means, if someone you see is misguided, then guiding that person to right path.

Sharing this is also a Sadaqah!
