CMT Academy for Different Licensure Examinations

The CMT Academy, initiated in 2024 by the Center of Manual Therapy, was founded by Dr. Dileep Kumar PT with the aim of helping physiotherapists successfully pass licensure examinations for various Middle Eastern countries, including the UAE, Oman, Qatar, Saudi Arabia, Bahrain, and more. This academy is designed specifically for physiotherapists, focusing on clearing the Prometric examination, which is required for practice in these regions. Success Rate & Achievements
Since its inception, the academy has launched 6 batches with a total of 35 candidates enrolled. Notably, all 35 candidates have achieved a 100% success rate, passing their respective exams on their first attempt. The breakdown of success across different countries is as follows:

• 25 candidates cleared the DHA (Dubai Health Authority) exam.
• 4 candidates passed the SLE (Saudi Licensing Exam).
• 2 candidates passed the DOH (Department of Health) exam in Abu Dhabi.
• 2 candidates cleared the Qatar exam.
• 1 candidate passed the Oman exam.

Introduction to the Diploma in Manual Therapy by the Center of Manual Therapy
The Diploma in Manual Therapy, initiated by the Center of Manual Therapy (CMT) in 2023, is an accredited program designed specifically for physiotherapists. This comprehensive course offers advanced training in manual therapy techniques and is ideal for those looking to specialize in the treatment of musculoskeletal conditions.

2 Batches have been launched already throughout Pakistan. Course Overview:

• Duration: 3 Months
• Total Classes: 21 (6 Physical & 15 Online)
• Physical Classes: Each physical class is 5 hours long and will be held at the Center of Manual Therapy in Karachi. For participants in other cities like Multan, Lahore, Quetta, and Hyderabad, the venue for physical classes will vary, but they can still join online classes and access recorded lectures.
• Online Classes: 15-17 online classes will be conducted via Zoom every Wednesday and Thursday. All international participants will have access to recorded lectures and online classes. Eligibility:

• The program is open to 3rd-year DPT students and graduated physiotherapists. Course Breakdown:

The curriculum covers over 300 manual therapy techniques, organized as follows:

• Month 1: 100 techniques for the Spine
• Month 2: 100 techniques for the Lower Extremity Joints
• Month 3: 100 techniques for the Upper Extremity Joints

Dry Needling for the Upper Trapezius Muscle: An Evidence-Based Approach

When performing (dry needling for the upper trapezius), the positioning of the muscle plays a critical role in both the effectiveness of the treatment and the comfort and safety of the patient. Clinical evidence suggests that proper muscle relaxation prior to needle insertion can significantly enhance the outcomes of dry needling interventions.

Although dry needling can be performed in various positions, it is important to understand that certain positions may be more advantageous depending on the muscle being targeted. In the case of the upper trapezius, performing the procedure in a sitting position is not necessarily incorrect, but it often fails to allow complete relaxation of the muscle fibers. This can lead to increased discomfort and reduced treatment efficacy.

Instead, research and clinical experience support the use of the prone lying position for dry needling of the upper trapezius. This position:

Promotes greater muscle relaxation
Reduces unnecessary tension in the muscle
Improves accuracy and ease for the therapist during needle insertion
Enhances patient comfort and decreases the likelihood of post-needling soreness

By choosing the optimal positioning, therapists can adhere more closely to evidence-based practice and provide safer, more effective care.

Reasons to Avoid Dry Needling & Release in Sitting Position

1. Increased Risk of Injury (especially pneumothorax)

Sitting posture may alter the angle of the neck and shoulder girdle.

The apex of the lung lies just beneath the upper trapezius, especially in thin individuals.

Improper needle angle or excessive depth can increase the risk of lung puncture, particularly when the scapula is not stabilized.

This is why some dry needling courses and guidelines prefer supine or prone needling for upper trapezius and levator scapulae.

2. Poor Muscle Stabilization

In sitting, the trapezius is active in postural support, especially upper and middle fibers.

Muscles are not fully relaxed, making it harder to localize trigger points or release fascial tension effectively.

3. Reduced Therapist Control

In prone or supine positions, the therapist can stabilize the scapula, head, or shoulder better.

In sitting, the patient may move, sway, or flinch, reducing precision during the procedure.

4. Patient Discomfort or Fatigue

Sitting for long periods—especially if the patient is already in pain—may lead to muscle guarding, limiting the effectiveness of dry needling or manual techniques.

Patients with cervical radiculopathy or dizziness may feel worse in unsupported sitting.

5. Biomechanical Disadvantage

Gravity affects blood flow, needling accuracy, and depth control differently in sitting vs. prone/supine.

It’s harder to palpate deeper fibers like those of the middle/lower trapezius or levator scapulae without full body relaxation.

Preferred Positions:

Prone → Full access to all trapezius fibers, scapula can be stabilized, and muscles are relaxed.

Supine → Safer for upper trapezius and neck muscles (especially when accessing near the clavicle).
