Patient was referred to me by one of the senior most general surgeon in Larkana. My input was, even staged pcnl (in two to three sessions) is better than open surgery (pyelolithotomy).

A Very Challenging Case Successfully Treated

A 5 year old child was diagnosed with a malrotated right kidney containing a large kidney stone.
This is a complex condition because abnormal kidney position makes surgery technically demanding and increases the risk of complications if not managed properly.

What was done

- Mini PCNL (Minimally Invasive Kidney Stone Surgery) was performed

- A very small incision of only 3 to 4 cm was required

- The stone was completely removed with 100 percent stone clearance

Key message

Even difficult pediatric kidney stone cases can be treated safely and effectively with the right expertise and modern technology.

Undiagnosed Posterior Urethral Valve (PUV) at 10 Years: What Parents Need to Know

Today I’m sharing a real case of a 10-year-old boy who was diagnosed very late with Posterior Urethral Valve (PUV) a condition that should ideally be picked up in infancy.

What is PUV?
PUV is a blockage in the tube that carries urine from the bladder out of the body. It happens only in boys and starts before birth.
This blockage makes it hard for the child to pass urine, causes the bladder to work under high pressure, and can gradually damage the kidneys.

Why Early Diagnosis Matters
PUV is one of the leading preventable causes of kidney failure in children.
When diagnosed early (preferably in infancy), we can:
• protect kidney function
• prevent repeated UTIs
• avoid bladder damage
• allow normal growth and development

But when diagnosis is delayed, like in this 10-year-old, the kidneys and bladder may already be significantly affected.

⚠️ Watch for These Warning Signs
• Recurrent urine infections
• Weak urine stream or straining
• Bedwetting or daytime wetting
• Swollen kidneys seen on ultrasound
• Child taking too long in washroom

✔️ Good News:
PUV is treatable, and timely treatment can make a big difference in outcomes.

3 year old child with 2cm kidney stone. Alot of pediatric surgeons and some urologists adviced open pyelolithotomy surgery. This is cruel, please avoid open surgery at such tender age.

Stone removal using Holmium laser and flexible URS. Same day discharge.
