A 30 year old female reported with a complaint of inability to open mouth since 25 years. On the basis of clinical examination, radiographs and CT scans, she was diagnosed with complete bilateral bony TMJ ankylosis.
Previously operated twice.
The treatment plan included bilateral condylectomy with coronoidectomy through al kayet Bramley approach and inter positional grafting with temporalis myofascial flap.

Alhamdulliah active 50 mm mouth opening achieved and passive more then 35 mm.

Patient on physiotherapy.

Presenting a case of severely displaced bilateral mandibular fracture.

This patient was managed with open reduction and internal fixation at both sites. He was placed into intermaxillary fixation using eyelets, and both fracture sites were exposed via intraoral vestibular approaches

2 plates at each site were placed

Adenocarcinoma of the maxilla is a rare cancer originating from the glandular cells lining the maxillary sinus. It can be invasive and have a high risk of recurrence after surgery, potentially extending into the paranasal sinuses and even metastasizing to other parts of the body. Treatment usually involves a combination of surgery, radiation therapy, and sometimes chemotherapy, with prognosis varying depending on the stage and location of the tumor.

Odontogenic myxoma is a rare intraosseous neoplasm, which is benign but locally aggressive. It rarely appears in any bone other than the jaws. It is considered to be derived from the mesenchymal portion of the tooth germ. Clinically, it is a slow-growing, expansile, painless, non-metastasizing, central tumor of jaws, chiefly the mandible
Radiographically, the classic presentation is that of a multilocular radiolucency, with well-developed locules, consisting of fine trabeculae, arranged at right angles, known as the ‘Tennis-racquet’ or ‘step-ladder’ pattern. A ‘sun-ray’ or ‘sun-burst’ appearance has also been reported in the literature

Treatment is mostly surgical involving the removal of the entire involved part of the jaw(resections).

Here we present u the case of biopsy proven odontogenic myxoma managed with segmental resection followed by reconstruction with free fibula(leg bone) to reconstruct the removed part of jaw.

The most common type of ameloblastoma is aggressive, forming a large tumor and growing into the jawbone. Treatment may include surgery and radiation. In some cases, reconstruction may be necessary to restore your teeth, jaw and facial appearance. Some types of ameloblastoma are less aggressive.

Here we present you a case of middle age male patient with solid ameloblastoma treated with segmental resection followed by reconstruction.

Needle fracture during dental local anesthetic injections is a rare but significant, potentially serious complication. Various approaches for the location and removal of broken needles have been described; however, there are several difficulties and concerns related to the potential complications and critical anatomic challenges peculiar to the head and neck region.

Here we are presenting you a case where we have used different diagnostic approaches utilized in locating a broken needle that migrated in the pterygomandibular space following sudden head movement of a middle-aged male patient at a general practice.

A meticulous diagnostic approach was employed to locate the needle utilizing CBCT scan, CT scan with contrast, and C-arm X-ray machine.

The needle was successfully retrieved using an angled hemostat inserted through an oral incision, guided by a C-arm X-ray machine

The zygoma is the main buttress between the maxilla and the skull, but, in spite of its sturdiness, its prominent location makes it prone to fracture. The mechanism of injury usually involves a blow to the side of the face from a fist, from an object, or secondary to motor vehicle accidents. Moderate force may result in minimally or nondisplaced fractures at the suture lines. More severe blows frequently result in inferior, medial, and posterior displacement of the zygoma. Comminuted fractures of the body with separation at the suture lines are most often the result of high-velocity motor vehicle accidents
The coronal incision with its various modifications provides the most versatile approach to various areas in the craniomaxillofacial region coupled with excellent exposure. The aesthetic advantage of a hidden scar in the hairline, accounts for its continued popularity.

On 25th April 2024, the Department of ORAL and Maxillofacial Surgery DIKIOHS/DUHS conducted a workshop on tips and tricks on mandibular trauma.

Around 30 residents from Karachi participated in this workshop and were supervised Individually by some renowned maxfac surgeons of Karachi.

Residents were allowed to perform plating on acrylic mandibles along with various transosseous wiring techniques.

Special thanks to the DIKIOHS OMFS team led by prof Anwar Ali and our guest master trainers Prof. Jehan Alam, Prof Zahid Ali, Dr Shaheen Ahmed

Overall the event was a huge success and we wish to continue this collaboration in the future with more workshops to help the maxfac residents.
